Glidil (Glimepiride)

Glidil (glimepiride) is indicated for the oral management of hyperglycemia in patients with type 2 diabetes mellitus when diet, exercise, and other non‑pharmacologic measures are insufficient.

• Used as monotherapy in patients with mild to moderate hyperglycemia.
• Added to metformin or other oral antidiabetic agents when additional glucose lowering is needed.
• May be prescribed in combination with lifestyle modifications to achieve individualized glycemic targets.
• Considered for patients who retain some endogenous insulin production.
• Not indicated for type 1 diabetes or diabetic ketoacidosis.

Glidil may cause a range of adverse effects, most of which are related to its glucose‑lowering action and to sulfonylurea‑specific reactions.

Common

• Mild hypoglycemia, especially if meals are missed or delayed.
• Nausea or abdominal discomfort.
• Headache or dizziness.
• Weight gain due to increased insulin activity.
• Transient flushing or warmth sensation.

Serious

• Severe hypoglycemia with neuroglycopenic symptoms (confusion, seizures, loss of consciousness).
• Allergic reactions such as rash, pruritus, or angio‑edema.
• Hematologic abnormalities like leukopenia or thrombocytopenia.
• Hepatic dysfunction manifested by jaundice or elevated liver enzymes.
• Pancreatitis presenting with abdominal pain and elevated enzymes.

When prescribing Glidil, clinicians should evaluate patient-specific factors and monitor for conditions that may increase the risk of adverse outcomes.

• Assess renal and hepatic function before initiation and periodically thereafter.
• Use lower starting doses in elderly patients or those with reduced kidney function.
• Advise patients to maintain regular meal patterns to avoid hypoglycemia.
• Caution in patients with a history of cardiovascular disease or arrhythmias.
• Avoid use during pregnancy and lactation unless benefits clearly outweigh risks.
• Monitor for signs of skin reactions or blood dyscrasias during therapy.

Glidil can interact with several medications that affect blood glucose levels or sulfonylurea metabolism.

Avoid

• Concurrent use with other sulfonylureas or insulin that may precipitate severe hypoglycemia.
• Strong CYP2C9 inhibitors (e.g., fluconazole, amiodarone) that increase glimepiride plasma concentrations.
• MAO‑B inhibitors that may potentiate hypoglycemic effects.

Use with caution

• Beta‑blockers, which can mask hypoglycemia symptoms.
