Glucobay (Acarbose)

Glucobay is indicated for the oral treatment of adults with type 2 diabetes mellitus when diet, exercise, and other oral agents are insufficient to control postprandial glucose.

• As monotherapy or adjunct to other oral antidiabetic agents to improve post‑meal glucose control.
• In patients who experience significant postprandial hyperglycemia despite metformin therapy.
• To be combined with lifestyle modifications, including diet and physical activity, for comprehensive glycemic management.
• For use in patients whose renal function is stable, as acarbose is minimally absorbed.
• May be considered when weight gain from other agents is a concern.

Glucobay may cause a range of side effects, most of which are related to its gastrointestinal action.

Common

• Flatulence or gas.
• Abdominal discomfort or cramping.
• Diarrhea.
• Soft or oily stools.
• Nausea.
• Mild hypoglycemia when combined with other hypoglycemic agents.

Serious

• Severe hypoglycemia, especially if meals are skipped.
• Liver enzyme elevations.
• Allergic reactions such as rash or urticaria.
• Acute pancreatitis (rare).
• Persistent or worsening gastrointestinal symptoms that lead to dehydration.

Before initiating Glucobay, certain precautions should be considered to ensure safe use.

• Assess renal and hepatic function; dose adjustment may be needed in impairment.
• Monitor blood glucose regularly, especially when combined with insulin or sulfonylureas.
• Caution in patients with inflammatory bowel disease or chronic intestinal disorders.
• Advise patients to take the tablet with the first bite of a meal to reduce gastrointestinal side effects.
• Pregnant or breastfeeding women should only use Glucobay if clearly indicated by a physician.
• Discontinue if severe gastrointestinal symptoms or signs of allergic reaction occur.

Glucobay can interact with several drug classes, influencing efficacy or safety.

Avoid

• Concurrent use with high‑dose sulfonylureas or insulin without dose adjustment, due to increased hypoglycemia risk.
• Combination with other alpha‑glucosidase inhibitors, which may amplify gastrointestinal adverse effects.

Use with caution

• Antacids containing aluminum or magnesium, which may reduce acarbose absorption.
• Digoxin, as delayed carbohydrate absorption can affect its plasma levels.
• Oral contraceptives, because altered gut absorption may affect hormone levels.
• Beta‑blockers, which can mask hypoglycemia symptoms.
• Other medications that require carbohydrate intake timing, such as certain antivirals.
