Glucosum (Glucose)

Glucosum is indicated for several clinical situations where rapid glucose supplementation or fluid replacement is required.

• Treatment of hypoglycemia when oral glucose is not feasible.
• Support of parenteral nutrition regimens as a carbohydrate source.
• Fluid and electrolyte replacement in patients with dehydration or postoperative fluid loss.
• Management of electrolyte imbalances during intravenous therapy.
• Adjunctive therapy during cardiac surgery or intensive care to maintain blood glucose levels.
• Use in emergency settings to quickly restore circulating glucose during trauma or severe infection.

Glucosum may be associated with a range of adverse effects, most of which are related to its osmolarity, rapid glucose infusion, or electrolyte content.

Common

• Transient increase in blood glucose levels (hyperglycemia) that usually resolves after the infusion is completed.
• Mild gastrointestinal discomfort such as nausea or occasional vomiting during or shortly after administration.
• Local irritation or erythema at the intravenous insertion site, especially with rapid infusion rates.
• Fluid overload, manifested as peripheral edema, in patients with pre‑existing cardiac or renal impairment.

Serious

• Severe hyperglycemia that may progress to osmotic diuresis, dehydration, and electrolyte loss if not monitored.
• Electrolyte disturbances, including hyponatremia, hyperkalemia, or shifts in calcium and magnesium balance.
• Acute hypotension caused by rapid infusion of a hypotonic solution, potentially leading to dizziness or syncope.

When using Glucosum, clinicians should observe specific precautions to minimize risks and ensure safe administration.

• Verify patient’s blood glucose level before initiating therapy to avoid excessive hyperglycemia.
• Adjust infusion rate in patients with cardiac failure, renal insufficiency, or risk of fluid overload.
• Monitor electrolytes (sodium, potassium, calcium) during and after the infusion.
• Use aseptic technique and inspect the solution for particulate matter or discoloration before administration.
• Document total glucose administered to prevent cumulative dosing errors.
• Consider compatibility with other IV drugs; avoid mixing Glucosum with medications that may precipitate or cause incompatibility.

Glucosum can interact with certain intravenous agents or conditions; awareness of these interactions helps maintain therapeutic safety.

Avoid

• Avoid concurrent infusion of high‑dose insulin without glucose monitoring, as this may cause severe hypoglycemia.
• Do not mix Glucosum with calcium‑containing solutions in the same line, which can precipitate.

Use with caution

• Use with caution when co‑administered with diuretics that affect electrolyte balance; monitor serum electrolytes closely.
• Caution is advised when Glucosum is given alongside beta‑blockers, as they may mask signs of hypoglycemia.
• When combined with hypertonic saline or other hyperosmolar solutions, adjust infusion rates to prevent fluid overload.
• Use carefully in patients receiving total parenteral nutrition, ensuring total carbohydrate load does not exceed prescribed limits.
