Glucovance (Metformin Hydrochloride, Glibenclamide)

Adults with type 2 diabetes mellitus who have not achieved adequate glycemic control with diet, exercise, and monotherapy using either metformin or a sulfonylurea may be considered for Glucovance, provided they have no contraindications such as severe renal impairment.

Glucovance combines both agents in a single tablet, which can simplify dosing schedules and improve adherence compared with taking two separate medications, while delivering the same pharmacologic actions of each component.

Common adverse effects include gastrointestinal symptoms (nausea, abdominal discomfort, diarrhea), mild hypoglycemia, a metallic taste, and modest weight gain or fluid retention.

Glucovance should not be used in patients with severe renal impairment, known hypersensitivity to metformin or sulfonylureas, or in situations where lactic acidosis risk is high, such as acute heart failure or significant liver disease.

Safety of Glucovance in pregnancy and lactation has not been established. Healthcare providers usually avoid sulfonylureas and may discontinue metformin during pregnancy unless benefits outweigh risks; breastfeeding decisions should be made with professional guidance.

Glucovance is typically taken with meals to reduce gastrointestinal discomfort and to coordinate the insulin‑stimulating effect of glibenclamide with food intake, helping to prevent hypoglycemia.

Regular monitoring of blood glucose levels, renal function (eGFR), liver enzymes, and signs of hypoglycemia is recommended. Periodic assessment of weight and cardiovascular status may also be advised.

Concurrent use of other sulfonylureas, insulin, iodinated contrast agents in patients with reduced renal function, and strong CYP2C9 inhibitors (e.g., fluconazole) should be avoided because they can increase the risk of severe hypoglycemia or toxicity.

Serious reactions include severe hypoglycemia (confusion, seizures, loss of consciousness), symptoms of lactic acidosis (muscle pain, rapid breathing, abdominal discomfort, unexplained fatigue), and allergic responses such as rash, swelling, or difficulty breathing. Prompt medical attention is required if these occur.

Elderly patients may have reduced renal function and increased sensitivity to hypoglycemia; therefore, clinicians often start with a lower dose and adjust cautiously while monitoring glucose and renal parameters.

Excessive alcohol intake can increase the risk of metformin‑associated lactic acidosis and may also affect blood glucose control, so patients are advised to limit alcohol while taking Glucovance.
