Glurenorm (Gliquidone)

Glurenorm (gliquidone) is indicated for the oral management of type 2 diabetes mellitus in adults. It may be used as monotherapy or in combination with other antidiabetic agents when glycemic targets are not met with lifestyle measures alone.

• Improves fasting and post‑prandial blood glucose levels.
• Can be combined with metformin, thiazolidinediones, or DPP‑4 inhibitors under medical supervision.

Common

• Mild hypoglycemia, especially if meals are delayed or missed.

• Skin reactions including rash or pruritus.
• Weight gain due to increased insulin activity.

Serious

• Severe hypoglycemia with neuroglycopenic symptoms (confusion, seizures, loss of consciousness).
• Allergic reactions such as angioedema or Stevens‑Johnson syndrome.
• Hepatic dysfunction manifested by elevated transaminases.
• Hematologic abnormalities like leukopenia or thrombocytopenia.

Before initiating Glurenorm, clinicians should evaluate patient history and laboratory parameters to minimize risk of adverse outcomes.

• Assess renal and hepatic function; dose adjustment may be required in impairment.
• Verify that the patient has adequate pancreatic beta‑cell reserve.
• Caution in elderly patients due to increased susceptibility to hypoglycemia.
• Avoid use during acute illness, fasting, or excessive alcohol consumption.
• Review concomitant medications that may potentiate hypoglycemia.
• Counsel patients on recognizing early signs of low blood sugar.
• Discontinue if recurrent severe hypoglycemia occurs.

Glurenorm can interact with several drug classes, altering its hypoglycemic effect or increasing the risk of adverse reactions.

Avoid

• Other sulfonylureas or meglitinides (additive hypoglycemia risk).
• Strong CYP2C9 inhibitors (e.g., fluconazole, amiodarone) that may raise gliquidone levels.
• High‑dose salicylates (enhanced insulin release).

Use with caution

• Beta‑blockers, which may mask hypoglycemia symptoms.
• ACE inhibitors or ARBs, which can potentiate hypoglycemia in renal impairment.
• Thiazide diuretics, which may affect glucose tolerance.
