Glypvilo (Vildagliptin)

Vildagliptin under the brand name Glypvilo is indicated for the management of type 2 diabetes mellitus in adults when diet, exercise, and other oral antidiabetic agents do not provide adequate glycemic control.

• As monotherapy for patients newly diagnosed with type 2 diabetes who require additional glucose lowering.
• In combination with metformin when metformin alone does not achieve target HbA1c.
• Added to a sulfonylurea regimen to enhance insulin secretion in patients inadequately controlled.
• Used together with insulin in patients who need further reduction of fasting glucose.
• Considered for patients with mild to moderate renal impairment when dose adjustment is appropriate.

Like other DPP‑4 inhibitors, Glypvilo may cause a range of adverse reactions. These are usually mild to moderate, but some can be serious and require medical attention.

Common

• Upper respiratory tract infection, such as nasopharyngitis.
• Headache, often mild and transient.
• Dizziness or light‑headedness, especially after initiating therapy.
• Gastrointestinal discomfort, including nausea or abdominal pain.

Serious

• Pancreatitis, presenting with severe abdominal pain and elevated enzymes.
• Severe allergic reactions, including angioedema or anaphylaxis.
• Hepatic dysfunction, indicated by marked elevation of liver transaminases.

Before starting Glypvilo, clinicians should evaluate patient history and monitor specific clinical parameters to minimize risks. Certain conditions require dose adjustment or avoidance of the drug, and consider alternative therapy when necessary.

• Assess renal function; dose may need reduction in moderate impairment.
• Review hepatic function; avoid use in severe liver disease.
