Gopten (Trandolapril)

Gopten is indicated for several cardiovascular and renal conditions where ACE inhibition is beneficial.

• Management of hypertension to achieve target blood pressure levels.
• Treatment of chronic heart failure to improve symptoms and functional capacity.
• Adjunct therapy in diabetic nephropathy to slow progression of kidney damage.
• Post‑myocardial infarction therapy to reduce remodeling risk.
• Use in combination with other antihypertensives when monotherapy is insufficient.

Common

• Cough, often dry and persistent.
• Dizziness or light‑headedness, especially after standing.
• Headache of mild to moderate intensity.

• Elevated blood potassium levels (hyperkalaemia).
• Reduced kidney function markers in laboratory tests.

Serious

• Angioedema presenting as facial, lip, tongue or throat swelling.
• Severe hypotension leading to fainting or shock.
• Acute kidney injury with rapid rise in serum creatinine.
• Persistent high potassium levels causing cardiac arrhythmias.
• Hepatic dysfunction with jaundice or abnormal liver enzymes.
• Allergic skin reactions such as Stevens‑Johnson syndrome.

Before starting Gopten, certain health factors should be evaluated to ensure safe use.

• Assess renal function; dose adjustment may be required in impairment.
• Monitor serum potassium, especially in patients on potassium‑sparing diuretics.
• Use with caution in patients with a history of angioedema or hereditary angioedema.
• Avoid use during the second and third trimesters of pregnancy (Category D).
• Evaluate for hypotension risk in volume‑depleted individuals.
• Review concomitant use of NSAIDs, which may affect kidney function.
• Conduct regular blood pressure checks to avoid excessive lowering.

Avoid

• Concurrent use with aliskiren in diabetic patients due to increased renal risk.
• Combination with other ACE inhibitors or ARBs, which may cause additive hypotension.

• Non‑steroidal anti‑inflammatory drugs (NSAIDs) that may impair renal function.
