Granisym (Granisetron)

Granisym is prescribed to manage nausea and vomiting in specific clinical settings. It is commonly used for:

• Prevention of chemotherapy‑induced nausea and vomiting (CINV).
• Prevention of postoperative nausea and vomiting (PONV).
• Adjunct therapy in patients receiving highly emetogenic chemotherapy.
• Situations where other antiemetics are insufficient or contraindicated.
• Supportive care for patients experiencing radiation‑induced nausea.

Granisym may cause a range of side effects, most of which are mild and transient.

Common

• Headache
• Constipation
• Fatigue
• Dizziness
• Diarrhea
• Abdominal discomfort

Serious

• Severe allergic reaction (rash, swelling, difficulty breathing)
• Cardiac arrhythmia or QT prolongation
• Persistent severe constipation or bowel obstruction
• Hepatic dysfunction indicated by abnormal liver enzymes

Before starting Granisym, certain precautions should be considered. Patients and clinicians should be aware of:

• Known hypersensitivity to granisetron or other 5‑HT3 antagonists.
• Pre‑existing cardiac conditions, especially those affecting QT interval.
• Severe hepatic impairment, which may require dose adjustment.
• Pregnancy and breastfeeding status, as safety data are limited.
• Concomitant use of other medications that may prolong the QT interval.
• Monitoring for signs of severe constipation or bowel obstruction.

Granisym can interact with other drugs, influencing its safety and effectiveness.

Avoid

• Other QT‑prolonging agents (e.g., certain antiarrhythmics, antipsychotics, macrolide antibiotics).
• Strong CYP3A4 inhibitors that may increase granisetron levels.

Use with caution

• CYP3A4 inducers such as rifampin, which may reduce efficacy.
• Drugs that also affect serotonin pathways (e.g., SSRIs, tramadol) due to additive effects.
• Medications causing electrolyte disturbances that can exacerbate QT prolongation.
• Concurrent use of other antiemetics, requiring dose adjustments.
