Heliclar (Clarithromycin)

Heliclar is indicated for several bacterial infections where clarithromycin is an appropriate therapy, and it may be used as monotherapy or as part of combination regimens.

• Community‑acquired pneumonia, including typical and atypical pathogens.
• Acute bacterial sinusitis caused by susceptible Streptococcus pneumoniae or Haemophilus influenzae.
• Acute bacterial exacerbations of chronic bronchitis in patients with confirmed infection.
• Skin and soft‑tissue infections such as cellulitis and erysipelas due to susceptible organisms.
• Part of combination therapy for Helicobacter pylori eradication in adults.
• Upper respiratory tract infections such as atypical mycoplasma pneumonia when susceptibility is confirmed.

The safety profile of Heliclar reflects the typical adverse events associated with macrolide antibiotics, most of which are mild and reversible, while rare serious events require prompt medical attention.

Common

• Gastrointestinal upset such as nausea, vomiting, abdominal pain, or diarrhea, usually mild and transient.
• Altered taste (metallic or bitter) and loss of appetite.
• Headache, dizziness, or mild central nervous system discomfort.
• Skin reactions including rash, pruritus, or mild erythema.

Serious

• Severe hypersensitivity reactions, including anaphylaxis, Stevens‑Johnson syndrome, or angioedema.
• Clostridioides difficile–associated colitis, which may present with profuse watery diarrhea.
• Cardiac effects such as QT interval prolongation that can precipitate torsades de pointes or other arrhythmias.

When prescribing Heliclar, clinicians should consider several precautionary measures to minimize risk and ensure appropriate use.

• Assess patient history for hepatic impairment; dose adjustment may be needed.
• Use caution in patients with known QT prolongation or cardiac arrhythmias.
• Evaluate for potential drug interactions, especially with medications that affect CYP3A4.
• Monitor for signs of Clostridioides difficile infection during and after therapy.
• Avoid use in patients with a documented hypersensitivity to macrolides or related antibiotics.
• Consider pregnancy category B status; discuss benefits versus risks with pregnant women.

Heliclar can interact with other medicines, and certain combinations should be avoided or used only with careful monitoring.

Avoid

• Co‑administration with strong CYP3A4 inhibitors such as clarithromycin, ketoconazole, or ritonavir, which can markedly increase clarithromycin levels.
• Concurrent use with drugs that prolong the QT interval (e.g., quinolones, antipsychotics, certain antiarrhythmics) due to additive cardiac risk.
• Combining with ergot alkaloids (e.g., ergotamine, dihydroergotamine) because of increased risk of ergot toxicity.

Use with caution

• Use with warfarin or other anticoagulants; clarithromycin may enhance anticoagulant effect, requiring INR monitoring.
• Concurrent use with statins metabolized by CYP3A4 (e.g., simvastatin, lovastatin) may increase statin concentrations and risk of myopathy.
• Administration with oral contraceptives may reduce contraceptive efficacy; advise alternative or additional contraceptive methods.
