Hermix (Aciclovir)

Hermix is indicated for the topical management of several herpes virus infections affecting the skin and mucous membranes.

• Treatment of herpes labialis (cold sores) on the lips or perioral area.
• Local therapy of external genital herpes lesions.
• Relief of pain and promotion of healing in herpes zoster (shingles) skin eruptions.
• Management of recurrent herpes simplex infections on the face or neck.
• Adjunctive care for post‑herpetic erythema or crusted lesions.

• Aciclovir is a nucleoside analogue that, after cellular uptake, is phosphorylated by viral thymidine kinase to aciclovir monophosphate and subsequently converted by host kinases to the active triphosphate form. The aciclovir triphosphate competitively inhibits viral DNA polymerase and becomes incorporated into viral DNA, causing chain termination and preventing further viral DNA synthesis. This selective action limits replication of herpes simplex and varicella‑zoster viruses in infected cells.

Topical aciclovir may cause local skin reactions, although many users experience no adverse effects.

Common

• Mild burning or stinging sensation at the site of application, usually lasting a few minutes.
• Transient itching or localized redness that resolves without treatment.
• Dryness, flaking or mild peeling of the skin where the cream is applied.
• Slight swelling or irritation that typically improves within several hours.

Serious

• Allergic contact dermatitis presenting as widespread rash, vesicles, or hives.
• Severe skin ulceration or necrotic lesions at the treated area.
• Persistent swelling, blister formation, or secondary bacterial infection.
• Systemic hypersensitivity reactions such as fever, malaise, or lymphadenopathy (rare).

When using Hermix, certain precautions should be observed to minimize risks and ensure safe application.

• Avoid applying to eyes, mucous membranes, or broken skin unless directed by a healthcare professional.
• Perform a test patch on a small skin area before full use to detect possible sensitivity.
• Discontinue use and seek medical advice if signs of allergic reaction develop.
• Keep the product out of reach of children; accidental ingestion may cause systemic effects.
• Store Hermix at room temperature, protected from moisture and direct sunlight.
• Do not use concurrently with other topical antiviral agents without professional guidance.

Topical aciclovir can interact with certain substances; awareness of these interactions helps prevent reduced efficacy or increased irritation.

Avoid

• Avoid simultaneous use with other topical antivirals unless prescribed, as combined irritation may increase.
• Avoid applying on skin pre‑treated with harsh chemical peels or abrasive scrubs, which can enhance absorption and irritation.

Use with caution

• Use with caution when combined with topical corticosteroids; steroids may mask local reactions.
• Use with caution if the patient is using systemic aciclovir or valacyclovir, to avoid duplicate therapy.
• Use with caution when applying near ophthalmic preparations, to prevent accidental eye exposure.
