Herpesedermyl (Aciclovir)

Herpesedermyl is indicated for the topical management of infections caused by herpes simplex virus.

• Primary (first‑time) herpes labialis (cold sores) on the lips or perioral skin.
• Recurrent herpes labialis episodes to shorten lesion duration.
• Herpes simplex infections of the facial skin, including eczema herpeticum.
• Herpes simplex lesions on the genital area when a topical option is preferred.
• Minor herpetic keratitis or conjunctivitis when applied under ophthalmic guidance.

• Aciclovir, the active component of Herpesedermyl, is a nucleoside analogue that is selectively phosphorylated by viral thymidine kinase after topical absorption. The resulting aciclovir monophosphate is further converted by cellular kinases to the triphosphate form, which competitively inhibits viral DNA polymerase. This blockade prevents elongation of viral DNA chains, thereby halting replication of herpes simplex virus in the treated tissue.

Local application of Herpesedermyl may cause skin reactions; most are mild and reversible, while rare events can be more serious.

Common

• Mild burning or stinging sensation at the site of application, usually lasting minutes.
• Transient itching or tingling that resolves without treatment.
• Slight redness or erythema confined to the treated area.
• Dryness, flaking or mild peeling of the skin.

Serious

• Severe allergic dermatitis characterized by swelling, blister formation, and intense itching.
• Contact urticaria or a widespread rash indicating a systemic hypersensitivity reaction.
• Persistent ulceration or necrosis of the skin that does not heal promptly.
• Signs of secondary bacterial infection such as pus formation, increased pain, or fever.

Before using Herpesedermyl, certain safety considerations should be reviewed to minimize risks and ensure appropriate use.

• Avoid application on broken, inflamed, or infected skin not related to herpes lesions.
• Do not use on mucous membranes, eyes, or genital mucosa unless directed by a healthcare professional.
• Perform a test application on a small skin area if a history of sensitivity to aciclovir exists.
• Discontinue use and seek medical advice if signs of an allergic reaction develop.
• Keep the product out of reach of children and store at room temperature away from excess heat.
• Consult a physician before use during pregnancy, lactation, or in immunocompromised individuals.

Topical aciclovir generally has limited systemic interactions, but certain substances and conditions may affect its safety or efficacy.

Avoid

• Concurrent use of other topical antiviral agents on the same area without professional guidance.
• Application together with irritant or drying skin products such as strong corticosteroids or retinoids.

Use with caution

• Systemic aciclovir or valacyclovir therapy, as combined antiviral activity may increase local irritation.
• Use on skin pre‑treated with chemical peels or laser therapy, which can enhance absorption.
• Application in patients receiving immunosuppressive therapy, where healing may be delayed.
• Use in conjunction with topical antibiotics, monitoring for signs of bacterial overgrowth.
