Herpesin (Aciclovir)

Herpesin (aciclovir) is indicated for the treatment and suppression of several herpes‑virus infections.

• Treatment of primary and recurrent genital herpes simplex infections.
• Management of oral herpes (herpes labialis) episodes.
• Therapy for herpes zoster (shingles) to reduce pain and accelerate lesion healing.
• Treatment of varicella (chickenpox) in immunocompetent patients.
• Post‑exposure prophylaxis for susceptible individuals after contact with herpes simplex virus.

• Aciclovir is a synthetic analogue of the nucleoside guanosine. After oral absorption, it is phosphorylated by viral thymidine kinase to aciclovir monophosphate, then further converted by cellular kinases to the active triphosphate form. Aciclovir triphosphate competitively inhibits viral DNA polymerase and becomes incorporated into viral DNA, causing chain termination and preventing viral replication without significantly affecting host DNA synthesis in infected cells.

Adverse reactions to Herpesin are generally mild, but rare serious events have been reported, and patients should be aware of potential signs.

Common

• Nausea or vomiting, usually transient and mild.
• Diarrhea, which may be watery and resolve without treatment.
• Headache, often mild and improves with rest.
• Mild skin rash, such as erythema or pruritus.
• Transient dizziness or light‑headedness.
• Mild abdominal discomfort.

Serious

• Acute kidney injury, particularly in patients receiving high doses, with dehydration, or concomitant nephrotoxic drugs.
• Severe allergic reactions, including anaphylaxis, Stevens‑Johnson syndrome, or toxic epidermal necrolysis, requiring immediate medical attention.
• Hematologic abnormalities such as neutropenia or thrombocytopenia.

Before using Herpesin, certain precautions should be considered to ensure safe and effective therapy.

• Assess renal function, as dose adjustment may be needed in impaired kidneys.
• Use caution in patients with immunosuppression or active malignancy.
• Evaluate for known hypersensitivity to aciclovir or related compounds.
• Monitor for signs of dehydration, especially during acute infections.
• Consider pregnancy and breastfeeding status; discuss potential risks with a clinician.
• Review concomitant medications that may affect renal clearance or increase toxicity.
