Herpesmed (Aciclovir)

Herpesmed is indicated for the treatment and management of infections caused by herpes simplex virus type 1 (HSV‑1) and type 2 (HSV‑2), including mucocutaneous and ocular manifestations.

• Oral mucosal lesions (cold sores)
• Genital herpes outbreaks
• Herpetic keratitis (eye infection)
• Prevention of viral shedding in immunocompromised patients
• Post‑exposure prophylaxis after known exposure to HSV

• Aciclovir, the active component of Herpesmed, is selectively taken up by cells infected with herpes simplex virus and phosphorylated by viral thymidine kinase to aciclovir monophosphate. Cellular kinases then convert it to aciclovir triphosphate, which competitively inhibits viral DNA polymerase, leading to premature chain termination and preventing synthesis of viral DNA, thereby limiting viral replication.

Adverse reactions to Herpesmed are generally mild but can include more serious events that require medical attention.

• Acute renal impairment or failure

• Use caution in pregnancy; the safety category is not established, so discuss risks with a healthcare professional.
• Monitor for signs of central nervous system toxicity, especially at high doses or in patients with renal dysfunction.
• Consider potential immunosuppression; patients with weakened immune systems may require closer observation.
• Inform patients about the importance of staying hydrated to support renal clearance.

Herpesmed may interact with other medicines, affecting its efficacy or safety.

Avoid

• Concomitant use of nephrotoxic agents such as aminoglycosides or high‑dose NSAIDs, which can increase the risk of kidney injury.
• Probenecid, which reduces renal clearance and raises aciclovir plasma levels.
• Cimetidine, which may also elevate aciclovir concentrations.

Use with caution

• Other antiviral drugs (e.g., valacyclovir) due to overlapping toxicity profiles.
• Immunosuppressive therapies or chemotherapy agents that may alter infection risk.
• Live vaccines, as antiviral therapy could affect vaccine efficacy.
