Hexomedine (Hexamidine Diisetionate)

Hexomedine is indicated for topical antisepsis of the skin in a variety of minor conditions.

• Skin antisepsis before minor surgical or diagnostic procedures.
• Cleaning and protection of minor cuts, abrasions, and lacerations.
• Prevention of infection in superficial burns or skin irritations.
• Use as a pre‑injection skin preparation in clinical settings.
• Adjunctive cleansing of skin prior to placement of catheters or wound dressings.

• Hexamidine diisetionate exerts its antiseptic effect by interacting with microbial cell membranes. The cationic molecule inserts into the phospholipid bilayer, disrupting membrane integrity and increasing permeability. This leads to leakage of intracellular contents and loss of vital metabolic functions, resulting in rapid bactericidal activity against a broad range of Gram‑positive and Gram‑negative bacteria, as well as some fungi and protozoa.

Topical application of Hexomedine may be associated with local skin reactions, most of which are mild, transient, and resolve after the product is removed. These reactions are typically reversible after discontinuation of the product.

Common

• Mild burning or stinging at the application site.
• Transient erythema or redness.
• Slight itching or pruritus.
• Dryness or peeling of the treated skin.
• Slight swelling or edema at the site.
• Minor discomfort lasting less than an hour.

Serious

• Allergic contact dermatitis with swelling and vesicle formation.
• Urticaria or widespread hives.
• Angioedema affecting face or lips.
• Anaphylactic reaction requiring emergency care.
• Severe skin necrosis (rare).
• Systemic hypersensitivity symptoms such as fever or malaise.

When using Hexomedine, certain precautions should be observed to minimize the risk of adverse reactions and ensure safe application.

• Do not apply to large areas of broken or ulcerated skin.
• Avoid contact with eyes, mucous membranes, or internal tissues.
• Discontinue use if signs of allergic reaction appear.
• Keep out of reach of children; store at room temperature away from direct sunlight.
• Consult a healthcare professional before use during pregnancy, lactation, or in immunocompromised individuals.
• Do not cover the treated area with occlusive dressings unless directed by a clinician.

Hexomedine has a low potential for systemic drug interactions, but certain topical combinations may affect tolerability or efficacy.

Avoid

• Concurrent use with other topical antiseptics containing alcohol or iodine that may increase irritation.
• Application over skin pre‑treated with strong keratolytic agents (e.g., salicylic acid) that could enhance absorption.

Use with caution

• Use together with topical corticosteroids; monitor for reduced antimicrobial effect.
• Combine with wound dressings containing silver; assess for additive irritation.
• Apply before systemic antibiotics; no known antagonism but monitor infection response.
