Hexvix (Hexaminolevulinate)

Hexvix is indicated for the diagnostic evaluation of patients undergoing cystoscopic examination of the bladder.

• Detection of non‑muscle invasive bladder cancer during blue‑light cystoscopy
• Identification of carcinoma in situ of the bladder
• Adjunct to standard white‑light cystoscopy to improve lesion visibility
• Assessment of tumor recurrence in surveillance protocols
• Guidance for targeted biopsy sampling of suspicious areas

• Hexvix contains hexaminolevulinate, a prodrug that is taken up by urothelial cells and enzymatically converted to protoporphyrin IX. Malignant cells preferentially accumulate higher concentrations of protoporphyrin IX, which emits a red fluorescence when illuminated with blue light, allowing clinicians to visually differentiate cancerous tissue from normal bladder mucosa during cystoscopy.

Adverse reactions to Hexvix are generally related to local bladder irritation and systemic hypersensitivity.

Common

• Mild bladder discomfort or urgency
• Dysuria or burning on urination
• Transient hematuria
• Nausea or mild headache
• Temporary urinary frequency

Serious

• Severe allergic reaction, including anaphylaxis
• Prolonged bladder spasm or severe pain
• Persistent urinary retention requiring intervention
• Systemic photosensitivity leading to skin reactions
• Significant hemorrhage or infection of the urinary tract

Hexvix should be used only under the supervision of a qualified urologist familiar with intravesical procedures.

• Contraindicated in patients with known hypersensitivity to hexaminolevulinate or similar compounds
• Avoid use in the presence of active urinary tract infection or recent bladder instrumentation
• Pregnancy status is unknown; use only if the potential benefit justifies the risk
• Ensure proper blue‑light equipment is available and calibrated before administration
• Monitor patients for immediate adverse reactions during and after instillation
• Document any prior photosensitizing drug exposure before use

Limited data exist on drug interactions with Hexvix, but caution is advised when other agents may affect bladder mucosa or photosensitivity.

Avoid

• Concurrent use of systemic photosensitizing medications (e.g., porphyrins, certain antibiotics)
• Simultaneous intravesical administration of other diagnostic or therapeutic agents
• Exposure to strong ambient light immediately after the procedure

Use with caution

• Anticoagulant therapy that may increase risk of hematuria
• Drugs that alter urinary pH, potentially affecting prodrug conversion
• Agents that cause bladder irritation or inflammation
• CYP enzyme inducers that could theoretically affect prodrug metabolism
