• After intravesical administration, hexaminolevulinate is preferentially taken up by malignant urothelial cells, where it enters the heme biosynthetic pathway and is converted to protoporphyrin IX. The accumulated protoporphyrin IX acts as a photosensitizer that emits red fluorescence when excited by blue light (380–450 nm), allowing visual differentiation of cancerous tissue from normal bladder mucosa during cystoscopy. The selective accumulation results from higher metabolic activity of tumor cells compared with surrounding healthy tissue.
