Hibicet (Cetrimide, Chlorhexidine Gluconate)

Hibicet is indicated for topical antiseptic applications where reduction of microbial load is required.

• Cetrimide and chlorhexidine gluconate act by disrupting microbial cell membranes. Cetrimide, a quaternary ammonium compound, inserts into lipid bilayers, increasing permeability and causing leakage of intracellular contents. Chlorhexidine gluconate binds to bacterial cell walls and precipitates cytoplasmic proteins, leading to coagulation of cellular components. Together they provide rapid, broad‑spectrum bactericidal, fungicidal, and virucidal activity on the skin surface. The combined effect results in swift reduction of viable microorganisms.

Topical application of Hibicet may cause local skin reactions; most are mild and reversible. These reactions are generally localized and resolve after discontinuation of the product.

Common

• Mild burning or stinging at the application site.
• Transient redness or erythema.
• Slight itching or tingling sensation.
• Dryness or peeling of the skin.
• Mild swelling of the treated area.
• Temporary discoloration of the skin.
• Mild warmth sensation at the site.
• Temporary tingling feeling.

Serious

• Allergic contact dermatitis with swelling and blistering.
• Severe skin irritation or ulceration.
• Rare systemic hypersensitivity reactions.
• Anaphylactic reaction (rare).
• Significant tissue necrosis (very rare).
• Delayed hypersensitivity dermatitis.
• Severe allergic reaction with systemic involvement.

When using Hibicet, certain precautions should be observed to minimize the risk of irritation and ensure safe application.

• Avoid applying to broken skin, mucous membranes, or deep wounds.
• Do not use on patients with known hypersensitivity to cetrimide, chlorhexidine, or related compounds.
• Keep the product away from eyes; if accidental contact occurs, rinse thoroughly with water.
• Store at room temperature, away from direct sunlight and out of reach of children.
• Use only as directed; do not exceed the recommended frequency or duration of application.
• Discontinue use and seek medical advice if a severe reaction develops.

Hibicet may interact with other topical agents or systemic medications that affect skin integrity or immune response.

Avoid

• Concurrent use with other antiseptic or disinfectant products on the same area, which may increase irritation.
• Application over skin pre‑treated with strong irritants or chemical peels.
• Applying over areas treated with topical retinoids, which can increase skin sensitivity.

Use with caution

• Use with topical corticosteroids; steroids may reduce the antiseptic’s efficacy.
• Co‑administration with systemic antibiotics is generally safe but monitor for allergic cross‑reactivity.
• Application on patients receiving chemotherapy or immunosuppressive therapy; monitor skin closely.
• Use in combination with wound dressings containing silver; assess for additive antimicrobial effect.
• Concurrent use with other quaternary ammonium compounds may lead to cumulative toxicity.
• Use with topical retinoids; monitor for increased irritation.
