Hidonac (Acetylcysteine)

Hidonac is indicated for two primary therapeutic purposes in adult patients.

• Mucolysis in chronic obstructive pulmonary disease (COPD) and other conditions with thickened airway secretions.
• Facilitation of expectoration in bronchiectasis, cystic fibrosis, or pneumonia.
• Prevention of respiratory complications during anesthesia or mechanical ventilation.
• Treatment of acute acetaminophen (paracetamol) poisoning to protect the liver.
• Adjunctive therapy in radiocontrast‑induced nephropathy.

• Acetylcysteine acts by cleaving disulfide bonds within mucus glycoproteins, which lowers the viscosity and elasticity of sputum, making it easier to clear. In the context of acetaminophen toxicity, the compound serves as a precursor for intracellular glutathione synthesis, replenishing depleted stores and enabling conjugation of the toxic metabolite N‑acetyl‑p‑benzoquinone imine (NAPQI). This dual action underlies its mucolytic and antidotal properties in clinical practice.

Adverse reactions to Hidonac are generally related to its intravenous administration and the pharmacologic activity of acetylcysteine.

Common

• Nausea or vomiting.
• Mild rash or urticaria at the infusion site.
• Transient flushing or warmth sensation.
• Headache or dizziness.
• Elevated liver enzymes without clinical hepatitis.

Serious

• Anaphylactoid reactions including bronchospasm, hypotension, or angioedema.
• Severe skin reactions such as Stevens‑Johnson syndrome or toxic epidermal necrolysis.
• Acute respiratory distress or pulmonary edema.
• Significant hepatic failure in the setting of massive acetaminophen overdose.

Before initiating Hidonac therapy, clinicians should evaluate several safety considerations to minimize risk.

• Assess patient history for hypersensitivity to acetylcysteine or related compounds.
• Use caution in patients with asthma or reactive airway disease due to potential bronchospasm.
• Monitor vital signs and respiratory status during the infusion, especially in critically ill individuals.
• Perform baseline liver function tests when treating acetaminophen overdose and repeat as needed.
• Adjust infusion rate in patients with renal impairment to avoid accumulation.
• Avoid rapid bolus administration; administer over the recommended period.

Acetylcysteine may interact with other medicines, influencing efficacy or safety.

Avoid

• Concurrent use of nitroglycerin or other vasodilators that may exacerbate hypotension during infusion.
• Simultaneous administration of activated charcoal, which can reduce acetylcysteine absorption when given orally.

Use with caution

• Anticoagulants such as warfarin, because changes in liver function can affect coagulation parameters.
• Certain chemotherapeutic agents metabolized by glutathione pathways.
