Hidrasec (Racecadotril)

Hidrasec is indicated for the symptomatic treatment of acute watery diarrhea in patients who require rapid reduction of intestinal fluid loss.

• Racecadotril, the active component of Hidrasec, is an enkephalinase inhibitor that acts peripherally in the gastrointestinal tract. By blocking the enzyme neutral endopeptidase, it prevents the breakdown of endogenous enkephalins. The increased enkephalin levels activate opioid receptors on the intestinal mucosa, which reduces secretion of water and electrolytes into the lumen without altering intestinal motility, thereby significantly limiting stool output.

Like most antidiarrheal agents, Hidrasec may cause adverse reactions, most of which are mild and transient. These events are usually self‑limiting, but patients should be aware of the signs that may require medical attention.

Common

• Nausea or mild stomach discomfort, often transient and resolving without intervention.
• Headache, typically low‑intensity and short‑lasting.
• Dizziness or light‑headed sensation, especially after standing quickly.
• Flatulence or abdominal bloating, which may accompany reduced intestinal secretion.
• Dry mouth, occasionally reported during treatment.
• Mild fatigue or feeling of weakness, generally not interfering with daily activities.

Serious

• Allergic skin reactions such as rash, itching, or urticaria that may require medical evaluation.
• Severe abdominal pain or persistent vomiting, which could signal intestinal obstruction or other serious gastrointestinal events.

When using Hidrasec, certain precautions should be considered to ensure safe and effective use.

• Verify that diarrhea is acute and watery; avoid use in cases of dysentery or inflammatory bowel disease.
• Ensure adequate fluid and electrolyte replacement, especially in children and the elderly.
• Do not exceed the recommended duration of therapy, typically a few days, without medical advice.
• Consult a healthcare professional before use in patients with known hypersensitivity to racecadotril or related compounds.
• Use cautiously in patients with renal or hepatic impairment, as drug clearance may be altered.
• Discontinue the product and seek medical attention if diarrhea persists beyond 48–72 hours or worsens.

Hidrasec may interact with other medicines; awareness of potential interactions helps avoid reduced efficacy or increased risk.

Avoid

• Concomitant use with other antidiarrheal agents such as loperamide, which may lead to excessive reduction of gut motility.
• Co‑administration with opioid analgesics, as combined effects on intestinal secretion could be unpredictable.
• Avoid simultaneous use with cholinergic agents that increase intestinal secretions, as the combined effect may counteract the therapeutic action.

Use with caution

• Antihypertensive drugs, because fluid loss from diarrhea may alter blood pressure control.
• Diuretics, as changes in electrolyte balance could be amplified.
• Antacids containing aluminum or magnesium, which might affect the absorption of racecadotril.
• Caution when used with medications that are substrates of the same metabolic pathways, although racecadotril has limited hepatic metabolism.
