• Racecadotril acts as a peripheral enkephalinase inhibitor. By blocking the enzyme that degrades endogenous enkephalins, it increases the local concentration of these peptides in the intestinal mucosa. The elevated enkephalins bind to opioid receptors on enteric neurons, reducing secretory activity and promoting water and electrolyte absorption, thereby decreasing the volume of diarrheal output without altering gut motility. The effect is confined to the gastrointestinal tract, minimizing central nervous system exposure.
