• Racecadotril, the active component of Hidrasec, is a prodrug that is rapidly converted to the active metabolite thiorphan after oral absorption. Thiorphan selectively inhibits the enzyme intestinal enkephalinase (neutral endopeptidase), leading to accumulation of endogenous enkephalins in the gut lumen. The elevated enkephalins bind to opioid receptors on enteric neurons, decreasing cyclic AMP‑mediated chloride secretion and thereby reducing fluid loss without altering intestinal transit.

Adverse reactions reported with Hidrasec are generally mild and transient, but rare serious events have been described in clinical observations.

Common

• Nausea or mild stomach discomfort, usually occurring shortly after the first dose.
• Headache, generally low‑intensity and transient.
• Dizziness or light‑headedness, often resolved without intervention.
