Himavat (Atorvastatin, Ezetimibe)

Himavat is indicated for the treatment of several lipid disorders where lowering low‑density lipoprotein (LDL) cholesterol is clinically important.

• Primary hypercholesterolemia (familial or non‑familial) requiring additional LDL‑C reduction beyond statin monotherapy.
• Mixed dyslipidemia with elevated LDL‑C and triglycerides when combination therapy is appropriate.
• Heterozygous familial hypercholesterolemia in adults who need intensified lipid lowering.
• Patients with established atherosclerotic cardiovascular disease who require further LDL‑C lowering.

• Atorvastatin competitively inhibits HMG‑CoA reductase, the rate‑limiting enzyme in hepatic cholesterol biosynthesis, leading to up‑regulation of LDL receptors and increased clearance of circulating LDL particles. Ezetimibe selectively blocks the Niemann‑Pick C1‑like 1 (NPC1L1) transporter on enterocytes, reducing intestinal absorption of dietary and biliary cholesterol. The dual action results in a significantly synergistic overall decrease in plasma LDL‑C and total cholesterol levels.

Like all lipid‑lowering agents, Himavat may cause adverse reactions that range from mild to severe, and patients should be aware of the typical and less common events.

Common

• Headache, dizziness or fatigue.
• Gastrointestinal discomfort such as abdominal pain, nausea or diarrhea.
• Myalgia or mild muscle cramps without significant CK elevation.
• Elevated liver enzymes (ALT/AST) that are transient.
• Transient increase in blood glucose levels, especially in diabetic patients.

Serious

• Clinically significant myopathy or rhabdomyolysis with marked CK rise.
• Acute liver injury presenting with jaundice or severe ALT/AST increase.
• Severe allergic reactions including rash, angioedema or anaphylaxis.
• Gallstones or biliary colic reported rarely.
• Hepatic failure leading to coagulopathy in patients with pre‑existing liver disease.

When prescribing Himavat, clinicians should evaluate patient history and monitor specific parameters to minimize risk and ensure safe use.

• Assess baseline liver function tests; avoid use in patients with active hepatic disease.
• Review renal function, especially in severe impairment, as dose adjustment may be needed.
• Screen for a history of muscle disorders or unexplained CK elevation before initiation.
• Counsel women of childbearing potential about potential risks and advise effective contraception.
• Monitor lipid profile periodically to determine therapeutic response and need for dose modification.
• Evaluate for drug interactions, particularly with CYP3A4 inhibitors or strong statin‑boosting agents.

Avoid

• Strong CYP3A4 inhibitors (e.g., clarithromycin, itraconazole, HIV protease inhibitors) that raise atorvastatin levels.
• Fibrates such as gemfibrozil, which markedly increase the risk of rhabdomyolysis.
• Cyclosporine or tacrolimus, which can potentiate statin‑related muscle injury.

Use with caution

• Moderate CYP3A4 inhibitors (e.g., diltiazem, verapamil) requiring closer monitoring of liver enzymes.
