Hirudoid (Chondroitin Polysulfate)

Hirudoid cream is indicated for several superficial vascular and skin conditions where local antithrombotic or anti‑inflammatory action is desired.

• Superficial thrombophlebitis of small veins, especially in the lower limbs.
• Bruising (contusions) and hematoma resulting from trauma or surgical procedures.
• Symptomatic varicose veins with associated skin irritation or mild edema.
• Localized skin inflammation such as erythema, mild dermatitis, or post‑injury swelling.
• Adjunctive care for minor burns or abrasions where reduced capillary leakage is beneficial.

• Chondroitin polysulfate is a heparinoid that mimics the activity of endogenous heparin when applied to the skin. It binds to antithrombin III and inhibits several coagulation factors, thereby limiting fibrin formation in superficial vessels. Concurrently, it reduces leukocyte migration and stabilises capillary membranes, which decreases vascular permeability and the release of inflammatory mediators, producing a local anti‑inflammatory effect and helps to prevent further tissue swelling.

Topical application of Hirudoid may be associated with local skin reactions, most of which are mild, reversible, and do not require medical intervention.

Common

• Mild erythema or redness at the application site, usually resolving within a few hours.
• Transient itching or pruritus that may be relieved by gentle washing.
• Slight burning or stinging sensation during or shortly after application, often diminishing with continued use.
• Dryness or peeling of the treated skin, indicating mild irritation.

Serious

• Allergic contact dermatitis presenting as extensive rash, swelling, and vesicle formation.
• Urticaria or hives that develop rapidly and may spread beyond the treated area.
• Anaphylactic reaction (rare) characterized by systemic symptoms such as difficulty breathing, throat swelling, or hypotension.

When using Hirudoid cream, certain precautions help minimize the risk of irritation and ensure safe application.

• Do not apply to broken, ulcerated, or infected skin.
• Avoid contact with eyes, mucous membranes, or open wounds.
• Keep the product out of reach of children; accidental ingestion may cause systemic effects.
• Use with caution in pregnancy or lactation, as safety data are limited.
• Discontinue use and seek medical advice if persistent redness, swelling, or pain develops.

Topical chondroitin polysulfate can interact with other agents applied to the same area or with systemic drugs that affect coagulation, potentially altering its local antithrombotic effect or increasing skin irritation.

Avoid

• Concurrent use of other topical heparinoids or anticoagulant creams, which may compound antithrombotic activity and increase bleeding risk on the skin.
• Application over areas treated with potent corticosteroids without medical guidance, as this may mask signs of irritation or infection.

Use with caution

• Systemic anticoagulants (e.g., warfarin, direct oral anticoagulants) – monitor for any signs of excessive bruising or bleeding at the application site.
• Non‑steroidal anti‑inflammatory drugs (NSAIDs) applied locally – may enhance local irritation or affect wound healing.
• Skin products containing alcohol, menthol, fragrances, or other irritants – could increase the likelihood of dermatitis when used together.
