Hirudoid (Mucopolysaccharide Polysulfuric Acid Ester)

Hirudoid is indicated for several superficial conditions where inflammation, bruising, or impaired microcirculation are present.

• Bruising (ecchymosis) resulting from minor trauma.
• Superficial thrombophlebitis of small veins.
• Hematoma formation following injury.
• Inflammatory skin conditions such as mild dermatitis.
• Symptomatic relief of varicose veins (pain, swelling).

• Mucopolysaccharide polysulfuric acid ester acts as a topical heparinoid. When applied to the skin it binds to endothelial surfaces and inhibits several coagulation factors, notably thrombin and factor Xa, reducing fibrin formation. The compound also interferes with leukocyte adhesion and migration, diminishing inflammatory cell influx. By improving capillary permeability and microvascular blood flow, it promotes resorption of extravasated blood and reduces edema.

Adverse reactions to Hirudoid are generally mild, but occasional serious events have been reported.

Common

• Local skin irritation or burning sensation.
• Mild erythema at the application site.
• Pruritus (itching).
• Dryness or peeling of the treated skin.

Serious

• Allergic contact dermatitis with vesiculation.
• Severe urticaria or angio‑edema.
• Necrosis of the skin in rare cases.
• Systemic hypersensitivity reactions (e.g., anaphylaxis).

Before using Hirudoid, consider the following precautions to minimize risk and ensure appropriate use.

• Do not apply to broken skin, open wounds, or mucous membranes.
• Avoid use on areas with active infection or severe dermatitis.

Hirudoid may interact with other topical or systemic agents that affect coagulation or skin integrity.

Avoid

• Concurrent use of other topical heparinoids or anticoagulant creams.
• Application over areas treated with strong irritants, such as retinoids or chemical peels.
• Use together with systemic anticoagulants when large skin areas are treated, as systemic absorption could potentiate anticoagulation.

Use with caution

• Combining with corticosteroid creams may increase skin thinning.
• Use alongside other anti‑inflammatory topicals (e.g., NSAID gels) may augment local irritation.
• Patients on systemic heparin should monitor for bleeding if extensive application occurs.
