Histakut (Dimetindene Maleate)

Histakut is indicated for the management of several allergic conditions where histamine release contributes to symptoms. It is administered intravenously when rapid control of symptoms is required, typically in clinical settings.

• Reduction of wheals, itching and swelling in acute urticaria episodes.

• Dimetindene maleate exerts its antihistaminic effect by selectively binding to peripheral H1 histamine receptors, preventing histamine from activating these receptors on mast cells, endothelial cells and sensory nerves. The antagonism reduces vasodilation, increased vascular permeability and nerve stimulation that underlie allergic symptoms. In addition, dimetindene possesses mild anticholinergic activity, which can further diminish secretions and itching associated with histamine release and discomfort.

Adverse reactions to Histakut have been reported; most are mild and reversible, while a few may need medical attention. Frequency varies among patients.

Common

• Drowsiness or mild sedation, usually transient and resolving without treatment.
• Dry mouth and throat causing mild discomfort that improves after the drug clears.
• Headache or dizziness, generally brief and not requiring specific therapy.
• Nausea or mild gastrointestinal upset that subsides within a few hours.

Serious

• Severe allergic reaction (anaphylaxis) with rash, swelling, and breathing difficulty, needing emergency care.
• Cardiac arrhythmias or palpitations that may require monitoring or treatment.
• Marked hypotension causing dizziness, fainting, or shock, especially in patients with cardiovascular disease.
• Rare extrapyramidal symptoms such as tremor or rigidity, which should prompt neurologic evaluation.

When prescribing Histakut, clinicians should consider several precautionary measures to ensure safe use, especially in patients with underlying health conditions.

• Assess for known hypersensitivity to dimetindene or other H1 antagonists before administration.
• Use caution in patients with cardiovascular disease, arrhythmias, or uncontrolled hypertension.
• Monitor respiratory status in individuals with asthma or chronic obstructive pulmonary disease.
• Evaluate liver and kidney function, as impairment may affect drug clearance.
