• Ifosfamide exerts its antineoplastic activity through intracellular activation by hepatic cytochrome P450 enzymes to form the alkylating metabolite isophosphoramide mustard. This metabolite creates covalent cross‑links between DNA strands, interfering with DNA replication and transcription. The resulting DNA damage triggers cell‑cycle arrest and apoptosis in rapidly dividing tumor cells, while sparing most non‑dividing normal tissues. The drug’s cytotoxic effect is enhanced in hypoxic tumor environments, and its activity can be modulated by co‑administration of protective agents such as mesna.

Adverse reactions to Holoxan are observed in a proportion of patients receiving intravenous ifosfamide; they vary in frequency and severity.

Common

• Nausea and vomiting
• Fatigue or generalized weakness
• Decreased white blood cell count (leukopenia)
• Mild to moderate hemorrhagic cystitis (bladder irritation)

• Severe hemorrhagic cystitis leading to hematuria
