Holpemic (Clotrimazole)

Holpemic is indicated for the topical treatment of common superficial fungal infections affecting the skin. The clotrimazole formulation targets the causative organisms and helps relieve associated symptoms such as redness, scaling, and itching.

• Dermatophytosis (ringworm) caused by Trichophyton, Microsporum or Epidermophyton species.
• Cutaneous candidiasis, including intertriginous and mucocutaneous yeast infections.
• Tinea corporis, tinea cruris, and tinea pedis (athlete’s foot) infections.
• Pityriasis versicolor (fungal skin discoloration) caused by Malassezia species.
• Superficial fungal infections of the nails when applied to the nail bed and surrounding skin.

• Clotrimazole belongs to the azole class of antifungals and works by inhibiting the fungal enzyme lanosterol 14α‑demethylase, a key step in ergosterol synthesis. By blocking this enzyme, the drug disrupts the production of ergosterol, an essential component of fungal cell membranes, leading to increased membrane permeability and inhibition of fungal growth. The resulting impairment of membrane integrity hampers cell division and ultimately reduces the viability of the infecting organism.

Topical clotrimazole is generally well tolerated, but local skin reactions may occur. Adverse events are usually mild and limited to the site of application, although rare systemic reactions have been reported.

Common

• Mild burning, stinging or itching at the application site.
• Redness or erythema of the skin.
• Dryness or peeling of the treated area.
• Pruritus (itching) unrelated to infection.

Serious

• Severe allergic dermatitis with swelling, blistering or hives.
• Contact urticaria or angio‑edema.
• Widespread skin rash or exfoliative dermatitis.
• Signs of systemic hypersensitivity such as fever or malaise.

Before using Holpemic, consider the following precautions to ensure safe and effective treatment. The product is intended for external use only and should not be applied to damaged mucous membranes or open wounds.

• Avoid application on extensive skin areas or broken skin unless directed by a healthcare professional.
• Do not use on the eyes, ears, or genital mucosa without specific medical advice.
• Discontinue use and seek medical attention if a rash or severe irritation develops.
• Pregnant or breastfeeding individuals should consult a clinician before starting treatment, as safety data are limited.
• Children under the age recommended on the package should use the product only under adult supervision.
• If the infection persists or worsens after two weeks, medical evaluation is advised.

Clotrimazole applied to the skin has a low potential for systemic drug interactions, but certain topical products may affect its absorption or increase irritation risk.

Avoid

• Concurrent use of other topical antifungal agents containing the same active ingredient, which may increase local irritation.
• Application of occlusive dressings that trap moisture and enhance drug penetration beyond intended levels.

Use with caution

• Co‑application with corticosteroid creams, which may mask signs of infection or alter skin barrier function.
• Use alongside other topical irritants such as retinoids, benzoyl peroxide, or chemical peels.
• Combining with systemic azole antifungals is generally unnecessary for topical therapy but should be discussed with a clinician if both are prescribed.
