Hoppas (Treprostinil)

Hoppas is indicated for the management of pulmonary arterial hypertension (PAH), a progressive disease characterized by elevated pressure in the pulmonary arteries.

• Improves exercise capacity and functional class in patients with PAH.
• Reduces right‑ventricular strain by lowering pulmonary vascular resistance.
• Can be used as monotherapy or in combination with other PAH‑targeted agents.
• Administered continuously via an implanted or external infusion pump.

• Treprostinil is a synthetic analogue of prostacyclin that selectively activates the prostacyclin (IP) receptor on vascular smooth‑muscle cells. Receptor activation stimulates adenylate cyclase, increasing intracellular cyclic adenosine monophosphate (cAMP). Elevated cAMP leads to relaxation of pulmonary arterial smooth muscle, producing vasodilation, and also inhibits platelet aggregation and smooth‑muscle proliferation, thereby improving hemodynamics in pulmonary arterial hypertension. These actions collectively contribute to reversal of adverse vascular remodeling.

Like other prostacyclin analogues, treprostinil may cause a range of adverse reactions that vary in frequency and severity.

• Injection‑site pain or erythema

Serious

• Severe hypotension
• Pulmonary edema
• Thrombocytopenia or bleeding complications

Healthcare providers should evaluate patients carefully before initiating Hoppas and monitor them regularly during therapy.

• Assess baseline hemodynamics and right‑ventricular function.
• Use caution in patients with low systemic blood pressure or recent myocardial infarction.
• Monitor for signs of infection at the infusion site.
• Adjust dose gradually to minimize hypotension and flushing.
• Counsel patients on proper pump handling and catheter care.
• Avoid abrupt discontinuation to prevent rebound pulmonary hypertension.

Treprostinil can interact with other medicines, affecting its efficacy or safety profile.

Avoid

• Concomitant use with potent vasodilators (e.g., nitroglycerin) that may cause additive hypotension.
• Phosphodiesterase‑5 inhibitors at high doses without dose adjustment.
• Non‑steroidal anti‑inflammatory drugs that increase bleeding risk.

Use with caution

• Anticoagulants (warfarin, direct oral anticoagulants) due to potential enhanced antiplatelet effect.
• Epoprostenol or other prostacyclin analogues, which may amplify vasodilatory effects.
• CYP2C8 inhibitors (e.g., gemfibrozil) that could raise treprostinil plasma levels.
