Hornhautpflaster (Salicylic Acid)

Hornhautpflaster is used to manage localized hyperkeratotic skin changes that develop from pressure or friction. It is applied directly to the affected area after cleaning and drying the skin.

• Removal of callus on the feet or hands.
• Treatment of corns (clavus) on weight‑bearing areas.
• Softening of thickened skin associated with plantar hyperkeratosis.
• Adjunctive care for areas of skin that have become hardened after repeated irritation.
• Preparation of the skin before applying other topical agents for better absorption.
• Management of mild hyperkeratosis on elbows or knees due to chronic rubbing.

• Salicylic acid exerts its keratolytic effect by disrupting the intercellular cohesion of keratinocytes in the stratum corneum. The acid penetrates the hyperkeratotic plaque, lowers the pH, and solubilizes corneodesmosomes, which weakens the bonds that hold dead skin cells together. This promotes desquamation and facilitates the gradual removal of the thickened layer. In addition, the lowered pH enhances the activity of endogenous proteolytic enzymes that further break down keratin aggregates.

Topical application of salicylic‑acid‑based Hornhautpflaster may cause local skin reactions, most of which are mild, temporary, and resolve after the product is removed.

Common

• Mild erythema or redness that may appear around the patch, usually resolving within a few hours after removal.
• Slight itching, tingling, or a warm sensation at the site, which typically diminishes as the skin adjusts.
• Localized dryness, flaking, or peeling of the treated skin layer, often indicating the keratolytic action.

Serious

• Persistent or severe irritation or burning that continues for more than several hours after the plaster is taken off, requiring washing the area.
• Allergic contact dermatitis characterized by swelling, redness, and formation of small blisters around the application zone.
• Development of ulceration, tissue breakdown, or necrosis when the product is left in place longer than recommended or applied to compromised skin.

When using Hornhautpflaster, certain precautions help minimize adverse reactions and ensure safe use.

• Do not apply to healthy, intact skin; limit use to areas with hyperkeratotic lesions.
• Avoid use on mucous membranes, genitalia, or broken skin.
• Keep the plaster away from eyes, nostrils, and mouth.
• Limit the duration of contact according to product instructions, typically not exceeding 24‑48 hours.
• Discontinue use and seek medical advice if severe irritation, rash, or swelling develops.
• Store at room temperature, away from moisture and direct sunlight.

Salicylic‑acid preparations can interact with other topical agents or systemic medications; awareness of these interactions helps avoid reduced efficacy or increased irritation.

Avoid

• Concurrent use with other keratolytic or exfoliating products (e.g., glycolic acid, urea) on the same site, which may increase skin irritation.
• Application together with occlusive dressings that trap moisture, enhancing absorption and risk of systemic exposure.

Use with caution

• Use with topical antibiotics or antiseptics; monitor for signs of irritation.
• Use in patients taking systemic salicylates (e.g., aspirin) if large skin areas are treated, due to potential additive salicylate load.
• Apply near areas where other dermatologic agents (steroids, retinoids) are used; observe for altered drug absorption.
