Hospasol (Sodium Hydrogen Carbonate)

Hospasol is employed in clinical practice where rapid correction of acid–base balance is required. It is administered intravenously by qualified personnel in a controlled setting.

• Management of metabolic acidosis in peri‑operative or critical‑care patients.
• Treatment of severe hyperkalemia when used in conjunction with insulin therapy.
• Buffering of urine during certain diagnostic or therapeutic procedures.
• Stabilization of blood pH during cardiopulmonary bypass.

• Sodium hydrogen carbonate dissociates in plasma to sodium ions and bicarbonate ions. The bicarbonate acts as a physiological buffer, combining with excess hydrogen ions to form carbonic acid, which is then converted to carbon dioxide and water via the respiratory system. This shift reduces hydrogen ion concentration, raises extracellular pH, and helps restore normal acid–base homeostasis. The added sodium contributes to extracellular fluid volume and osmolarity.

Adverse reactions to Hospasol are generally related to its alkalinizing and sodium‑containing properties.

Common

• Mild hypernatremia.
• Transient metabolic alkalosis.
• Local irritation or discomfort at the injection site.
• Flushing or warmth sensation.
• Mild headache.

Serious

• Severe metabolic alkalosis leading to respiratory depression.
• Hypernatremia with volume overload and hypertension.
• Cardiac arrhythmias due to electrolyte shifts.
• Pulmonary edema in susceptible patients.
• Severe hypotension from rapid infusion.

Hospasol should be administered only after careful assessment of the patient’s acid–base status and electrolyte balance.

• Verify baseline serum bicarbonate, pH, and sodium levels before infusion.
• Adjust dose in patients with renal impairment or congestive heart failure.
• Monitor arterial blood gases and electrolytes during and after administration.
• Use caution in patients receiving other sodium‑containing solutions to avoid hypernatremia.
• Avoid rapid large‑volume infusions that may cause circulatory overload.
• Ensure proper aseptic technique to prevent infection at the injection site.
• Discontinue or adjust therapy if signs of metabolic alkalosis develop.

Concurrent administration of certain agents may alter the safety or efficacy of Hospasol.

Avoid

• Other alkalinizing agents (e.g., oral bicarbonate) that could precipitate severe alkalosis.
• High‑dose calcium or magnesium supplements that may exacerbate electrolyte imbalances.
• Medications that prolong the QT interval when severe alkalosis is present.

Use with caution

• Loop diuretics, which can affect sodium and potassium balance.
• ACE inhibitors or ARBs, especially in patients with renal dysfunction.
• Insulin therapy for hyperkalemia, as rapid shifts in potassium may occur.
• Intravenous fluids containing high sodium concentrations.
• Drugs with narrow therapeutic windows that are pH‑sensitive (e.g., certain antibiotics).
