Hulio (Adalimumab)

Hulio (adalimumab) is indicated for the treatment of several chronic inflammatory and autoimmune conditions where suppression of tumor necrosis factor‑alpha is beneficial.

• Rheumatoid arthritis – moderate to severe disease unresponsive to conventional DMARDs
• Psoriatic arthritis – active joint disease with skin involvement
• Ankylosing spondylitis – axial skeletal inflammation in adults
• Crohn’s disease – moderate to severe intestinal inflammation
• Ulcerative colitis – extensive disease requiring systemic therapy
• Plaque psoriasis – widespread plaques not controlled by topical agents
• Hidradenitis suppurativa – chronic nodular lesions in typical areas

• Adalimumab is a fully human monoclonal antibody that selectively binds to soluble and membrane‑bound tumor necrosis factor‑alpha (TNF‑α). By attaching to TNF‑α, it prevents the cytokine from interacting with its p55 and p75 cell surface receptors, thereby inhibiting the downstream NF‑κB signaling cascade that drives inflammation. This blockade reduces the recruitment of immune cells and the production of other pro‑inflammatory mediators in affected tissues.

Adalimumab may cause a range of adverse reactions, most of which are mild to moderate, while a smaller number can be serious and require medical attention.

Common

• Injection site pain, redness, swelling, or bruising
• Upper respiratory tract infections such as sinusitis, nasopharyngitis, or common cold
• Headache of mild to moderate intensity
• Nausea or decreased appetite
• Mild skin rash, itching, or localized urticaria
• Fatigue or general tiredness

Serious

• Serious infections including active tuberculosis, bacterial sepsis, opportunistic fungal infections, or viral reactivation
• Reactivation of chronic hepatitis B virus infection, which may lead to liver inflammation
• Malignancies such as lymphoma, leukemia, or non‑melanoma skin cancer observed during therapy
• Severe allergic reactions including anaphylaxis, angioedema, or serum sickness‑like syndrome

Before starting Hulio, patients and clinicians should consider several precautionary measures to minimize risks and ensure safe use.

• Screen for active or latent tuberculosis and treat before therapy.
• Evaluate for chronic infections such as hepatitis B, hepatitis C, or HIV and monitor during treatment.
• Assess for a history of demyelinating disease, congestive heart failure, or recent malignancy.
• Use caution in pregnant or breastfeeding women; safety data are limited.
• Monitor complete blood counts and liver function tests regularly while on therapy.

Adalimumab can interact with other medicines, affecting its efficacy or increasing the risk of adverse events.

Avoid

• Live attenuated vaccines (e.g., measles, varicella) because immunosuppression may cause infection.
• Concomitant use with other biologic agents targeting the immune system (e.g., abatacept, rituximab) due to heightened infection risk.
• Concurrent use with strong immunosuppressants such as high‑dose corticosteroids may increase infection risk.

Use with caution

• Methotrexate or other disease‑modifying antirheumatic drugs (DMARDs) – may increase immunosuppression.
• Antifungal agents such as azoles – may alter infection risk.
• Cyclosporine or tacrolimus – additive immunosuppressive effect.
• Interferon‑beta – potential for increased autoimmune reactions.
• Vaccination with inactivated vaccines – response may be reduced, timing should be considered.
