Hulio (Adalimumab)

Hulio is indicated for the treatment of several immune‑mediated inflammatory disorders where suppression of tumor necrosis factor‑alpha is clinically beneficial.

• Rheumatoid arthritis in adults, to reduce joint pain, swelling and structural damage.
• Psoriatic arthritis, to improve skin lesions and joint symptoms.
• Ankylosing spondylitis, to alleviate spinal inflammation and stiffness.
• Moderate to severe plaque psoriasis, to clear plaques and improve quality of life.
• Crohn’s disease, to induce and maintain remission of intestinal inflammation.
• Ulcerative colitis, to achieve clinical remission and mucosal healing.
• Hidradenitis suppurativa, to reduce painful nodules and abscess formation.

• Adalimumab is a fully human monoclonal antibody that selectively binds soluble and transmembrane forms of tumor necrosis factor‑alpha (TNF‑α). By attaching to TNF‑α, it prevents the cytokine from interacting with its cell‑surface receptors, thereby inhibiting downstream signaling pathways that drive inflammatory cell recruitment, activation, and tissue destruction. This neutralization reduces the cascade of pro‑inflammatory mediators involved in autoimmune disease processes.

Adalimumab may cause a range of adverse reactions, most of which are mild to moderate, while rare events can be serious. These events are reported in clinical studies and post‑marketing surveillance.

Common

• Injection‑site pain, redness or swelling.
• Upper respiratory tract infections such as nasopharyngitis.
• Headache.
• Nausea.
• Mild rash or itching.
• Fatigue.
• Diarrhea.
• Mild fever.
• Upper respiratory symptoms like sinusitis.

Serious

• Serious infections including tuberculosis, bacterial sepsis, or opportunistic fungal infections.
• Reactivation of hepatitis B virus.
• Malignancies such as lymphoma or skin cancer.
• Severe allergic reactions (anaphylaxis, serum sickness‑like syndrome).
• Autoimmune hepatitis or other immune‑mediated liver injury.
• Neurological events such as demyelinating disease.
• Heart failure exacerbation.

When prescribing Hulio, clinicians should evaluate patient history and monitor for conditions that could be worsened by TNF‑α inhibition.

• Active or latent tuberculosis must be screened and treated before initiation.
• Chronic infections such as hepatitis B or C require assessment and possible antiviral prophylaxis.
• History of malignancy, especially lymphoma, warrants careful risk‑benefit analysis.
• Congestive heart failure may worsen; avoid use in patients with NYHA class III/IV.
• Pregnancy and breastfeeding status should be discussed, as safety data are limited.
• Vaccinations should be up to date; live vaccines are contraindicated during therapy.
• Regular laboratory monitoring (CBC, liver enzymes) is recommended.

Adalimumab can interact with other medicines, influencing infection risk or immune response.

Avoid

• Live attenuated vaccines (e.g., measles, varicella) while receiving Hulio.
• Concurrent use of other biologic immunosuppressants (e.g., abatacept, rituximab) due to heightened infection risk.
• Systemic corticosteroids at high doses without medical supervision.
• Combination with JAK inhibitors should be avoided because of additive immunosuppression.

Use with caution

• Methotrexate or azathioprine may increase immunosuppression; monitor for infections.
• Antifungal agents such as azoles can affect infection susceptibility.
• Interferon‑beta therapy may alter disease activity; assess clinical response.
• Concomitant use of warfarin may require INR monitoring due to potential bleeding risk.
