Hydrex (Hydrochlorothiazide)

Hydrex is indicated for several conditions where reduction of excess fluid or control of blood pressure is needed.

• Management of primary hypertension.
• Treatment of edema associated with congestive heart failure.
• Reduction of fluid accumulation in patients with chronic kidney disease.
• Relief of ascites‑related swelling in liver cirrhosis.
• Adjunct therapy for hypertension when combined with other antihypertensive agents.

• Hydrochlorothiazide, the active component of Hydrex, blocks the Na⁺/Cl⁻ symporter located in the distal convoluted tubule of the nephron. By inhibiting this transporter, the drug reduces sodium and chloride reabsorption, leading to increased urinary excretion of NaCl and water. The resulting decrease in extracellular fluid volume lowers cardiac output and peripheral vascular resistance, contributing to a reduction in systemic blood pressure.

Hydrex may cause a range of adverse reactions, most of which are mild and transient, while less common events can be serious.

Common

• Increased urination (polyuria) and mild thirst.
• Low blood potassium (hypokalemia) leading to muscle cramps.
• Slight rise in blood glucose levels.
• Dizziness or light‑headedness, especially when standing.
• Elevated blood uric acid, potentially precipitating gout.

Serious

• Severe electrolyte disturbances such as hyponatremia or hypercalcemia.
• Acute kidney injury manifested by reduced urine output.
• Allergic reactions including rash, itching, or angioedema.
• Significant drop in blood pressure (hypotension) causing fainting.
• Pancreatitis or severe hepatic dysfunction (rare).

• Assess baseline renal function and electrolyte levels, especially potassium and sodium.
• Monitor blood pressure regularly to avoid excessive hypotension.
• Use caution in patients with a history of gout or elevated uric acid.
• Adjust dose or avoid use in individuals with severe liver disease or heart failure decompensation.
• Review concurrent medications that may affect potassium balance.
• Consider pregnancy status; the safety profile in pregnancy is not well established.
• Advise patients to rise slowly from sitting or lying positions to reduce orthostatic dizziness.

Hydrex can interact with other medicines, altering its diuretic effect or increasing the risk of adverse events.

Avoid

• Concomitant use with other thiazide or loop diuretics, which may cause profound electrolyte loss.
• Strong potassium‑depleting agents such as high‑dose corticosteroids or amphotericin B.

Use with caution

• Non‑steroidal anti‑inflammatory drugs (NSAIDs) that may reduce diuretic efficacy and affect renal function.
• ACE inhibitors, ARBs, or beta‑blockers, which can potentiate blood‑pressure lowering.
• Lithium, as thiazides may increase lithium concentrations and toxicity risk.
• Antidiabetic agents, because thiazides can raise blood glucose levels.
