Hydrocort (Hydrocortisone Acetate)

Hydrocort is indicated for the topical treatment of several inflammatory skin disorders where reduction of redness, swelling, and itching is desired, and it may be applied to affected areas once or twice daily as directed by a healthcare professional.

• Mild to moderate eczema (atopic dermatitis)
• Contact dermatitis caused by irritants or allergens
• Psoriasis plaques with limited surface area
• Seborrheic dermatitis of the scalp or face
• Minor inflammatory rashes such as insect bites
• Acute flare‑ups of chronic dermatitis

• Hydrocort delivers hydrocortisone acetate to the skin, where the ester is hydrolyzed to hydrocortisone. The active steroid binds to intracellular glucocorticoid receptors, forming a complex that translocates to the nucleus and modulates gene transcription. This interaction suppresses the production of pro‑inflammatory cytokines, reduces phospholipase A2 activity, and limits the migration of immune cells, resulting in decreased inflammation and immune response in the treated area.

Topical application of Hydrocort may be associated with local skin reactions and, less frequently, systemic effects, especially when used over large areas or for prolonged periods.

Common

• Mild burning or stinging at the application site
• Pruritus (itching) that may persist
• Dryness or peeling of the skin
• Erythema or mild redness
• Transient swelling at the site
• Mild itching that resolves after a few days

Serious

• Skin atrophy or thinning with prolonged use
• Telangiectasia (visible small blood vessels)
• Secondary bacterial, fungal or viral infection of the treated skin
• Hypersensitivity reactions such as contact dermatitis
• Systemic corticosteroid effects such as adrenal suppression (rare)
• Delayed wound healing

When using Hydrocort, certain precautions should be observed to minimize the risk of adverse reactions and to ensure safe and effective therapy.

• Avoid application to broken, infected, or ulcerated skin unless directed by a clinician.
• Limit use to the prescribed area and duration; prolonged therapy may increase systemic absorption.
• Do not use under occlusive dressings unless specifically instructed.
• Exercise caution in patients with a history of skin thinning, rosacea, or periorificial dermatitis.
• Discontinue and seek medical advice if signs of hypersensitivity develop.
• Pregnant or breastfeeding individuals should consult a healthcare professional before use.

Hydrocort may interact with other topical or systemic agents that affect skin integrity or corticosteroid metabolism.

Avoid

• Concurrent use of other potent topical corticosteroids on the same area.
• Application of topical antibiotics or antifungals that are not compatible with corticosteroid formulations.
• Use with systemic glucocorticoids when systemic exposure could be additive.

Use with caution

• Combining with retinoids or keratolytic agents, which may increase irritation.
• Use alongside phototherapy, as corticosteroids can alter skin response to UV light.
• Co‑administration with non‑steroidal anti‑inflammatory drugs (NSAIDs) applied topically, which may raise risk of local skin reactions.
