Hydrocortisone

Hydrocortisone topical preparations are employed to manage a range of skin disorders characterized by inflammation, itching, or allergic response. They are applied directly to affected areas to reduce redness, swelling, and discomfort.

• Mild to moderate eczema (atopic dermatitis)
• Contact dermatitis caused by irritants or allergens
• Psoriasis plaques with limited surface involvement
• Insect bite reactions with localized swelling
• Minor skin irritations such as rash or sunburn
• Seborrheic dermatitis of the scalp or face

• Hydrocortisone acts as an agonist at intracellular glucocorticoid receptors. After penetrating the epidermis, the molecule binds to the receptor complex, translocates to the nucleus, and modulates gene transcription. This results in decreased production of pro‑inflammatory cytokines, inhibition of phospholipase A2, and reduced expression of adhesion molecules, collectively dampening the inflammatory and immune response in the skin. The overall effect is a rapid reduction of edema, erythema, and pruritus at the site of application.

While generally well tolerated, hydrocortisone applied to the skin can produce adverse reactions ranging from mild irritation to rare systemic effects, especially with prolonged use or large surface areas.

Common

• Burning, stinging, or itching sensations at the site of application, usually transient.
• Dryness, flaking, or peeling of the treated skin area.
• Localized redness or mild irritation that resolves with continued use or discontinuation.
• Development of folliculitis or acne‑like eruptions on the applied region.

Serious

• Thinning (atrophy) of the skin with prolonged or excessive use, potentially leading to visible translucency.
• Formation of stretch marks (striae) in areas where the medication has been applied over time.
• Allergic contact dermatitis to the hydrocortisone preparation, presenting as intense rash and swelling.
• Suppression of the hypothalamic‑pituitary‑adrenal (HPA) axis when large body surface areas are treated, which may affect systemic cortisol levels.

Before using topical hydrocortisone, certain precautions should be considered to minimize risk and ensure appropriate therapy. Patients should assess skin condition, duration of treatment, and any underlying health issues, and follow labeling instructions.

• Avoid application to broken, infected, or ulcerated skin unless directed by a healthcare professional.
• Do not use on the face, groin, or axillae for more than a few days without medical supervision.
• Limit treatment duration, typically not exceeding two weeks, to reduce the risk of skin atrophy.
• Use the smallest effective amount and avoid covering the treated area with occlusive dressings unless advised.
• Consult a physician before use in children, pregnant or breastfeeding individuals, or patients with systemic steroid therapy.
• Discontinue use and seek medical advice if signs of allergic reaction or worsening skin condition occur.

Topical hydrocortisone can interact with other topical agents or systemic medications that affect steroid metabolism or skin integrity. Awareness of these interactions helps prevent reduced efficacy or increased adverse effects.

Avoid

• Concurrent use of other potent topical corticosteroids on the same area, which may increase systemic absorption.
• Application together with topical antibiotics that contain preservatives known to cause irritation, unless combined in a single formulation.

Use with caution

• Systemic glucocorticoids or oral steroid therapy, as combined effects may suppress the HPA axis.
• Antifungal creams or ointments, because altered skin barrier may affect hydrocortisone penetration.
• Phototherapy or UV light treatments, which can increase skin sensitivity when combined with corticosteroids.
