Hydrokortyzon Allefin Max (Hydrocortisone Acetate)

Hydrocortisone acetate is a topical corticosteroid used to manage inflammatory skin disorders. It provides anti‑inflammatory and antipruritic effects in the following conditions:

• Atopic dermatitis: alleviates redness, itching and swelling.
• Psoriasis: reduces plaque formation and scaling.
• Contact dermatitis: diminishes irritation from allergens or irritants.
• Seborrheic dermatitis: controls flaking and erythema on scalp and face.
• Insect bite reactions: eases local swelling and itching.

• Hydrocortisone acetate penetrates the epidermis and binds to intracellular glucocorticoid receptors. The drug‑receptor complex translocates to the nucleus, where it alters gene transcription, suppressing pro‑inflammatory cytokines, decreasing leukocyte migration, and stabilising lysosomal membranes, thereby producing anti‑inflammatory and immunosuppressive effects on the skin.

Topical hydrocortisone acetate can cause local and systemic adverse reactions, most of which are dose‑ and duration‑dependent.

Common Side Effects

• Burning or stinging sensation at the application site.
• Skin irritation or erythema.
• Dryness or peeling of the treated area.
• Pruritus or itching.
• Folliculitis or acneiform eruptions.

Serious or Rare Side Effects

• Skin atrophy or thinning with prolonged use.
• Telangiectasia (visible small blood vessels).
• Secondary bacterial, fungal or viral skin infection.
• Systemic adrenal suppression (rare with extensive application).
• Hypersensitivity reactions such as urticaria or angioedema.

When using hydrocortisone acetate topical formulations, consider the following safety measures:

• Prolonged use: limit duration to the shortest effective period to avoid skin atrophy.
• Infected skin: do not apply on active bacterial, fungal or viral infections unless directed by a clinician.
• Occlusion: avoid covering treated areas with airtight dressings unless prescribed.
• Pediatric use: use only under medical supervision in children, adjusting amount and duration.
• Sensitive areas: avoid application on face, groin or axillae unless specifically indicated.
• Store at room temperature: keep in the original container, protect from moisture and direct sunlight.

Hydrocortisone acetate may interact with other agents that affect skin absorption or systemic corticosteroid levels.

Major Interactions (Avoid)

• Systemic corticosteroids: concurrent use can increase risk of systemic side effects.
• Potent CYP3A4 inhibitors (e.g., ketoconazole cream): may raise local drug concentration.
• Occlusive dressings: can markedly enhance percutaneous absorption.

Moderate Interactions (Monitor Closely)

• Topical antibiotics or antifungals: may increase local irritation.
• UV light exposure: may heighten photosensitivity and skin damage.
• Other topical steroids: cumulative potency may lead to increased adverse effects.
