Hydroxy-Vit (Hydroxocobalamin)

Hydroxocobalamin is used to treat conditions related to vitamin B12 deficiency and associated hematologic disorders.

• Vitamin B12 deficiency: restores normal B12 levels and improves related symptoms.
• Pernicious anemia: corrects the underlying cobalamin lack and supports red blood cell production.
• Megaloblastic anemia due to B12 deficiency: promotes DNA synthesis and normalizes marrow activity.
• Malabsorption syndromes: provides a parenteral route to bypass gastrointestinal absorption defects.

• Hydroxocobalamin acts as a cofactor for the enzymes methionine synthase and methylmalonyl‑CoA mutase. By facilitating the conversion of homocysteine to methionine and the metabolism of odd‑chain fatty acids, it supports DNA synthesis, myelin formation, and normal red blood cell development, thereby correcting functional vitamin B12 deficiency.

Adverse reactions may occur following intramuscular administration of hydroxocobalamin.

Common Side Effects

• Pain or bruising at the injection site.
• Mild rash or erythema.
• Headache.
• Nausea.
• Dizziness.

Serious or Rare Side Effects

• Anaphylactic reaction.
• Severe hypotension.
• Hemolytic anemia.
• Stevens‑Johnson syndrome or toxic epidermal necrolysis.
• Acute renal impairment.

Hydroxocobalamin should be used with attention to patient history and handling requirements.

• Allergy assessment: avoid use in patients with known hypersensitivity to hydroxocobalamin or other cobalamin products.
• Renal impairment: monitor closely as reduced clearance may increase systemic exposure.
• Pregnancy: consider only if the potential benefit outweighs any unknown risk; data are limited.
• Concurrent cyanide exposure: hydroxocobalamin may interfere with certain laboratory assays.
• Administration technique: inject deep into muscle using aseptic technique.
• Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake.

Hydroxocobalamin may interact with other medications or substances that affect cobalamin metabolism.

Major Interactions (Avoid)

• Sodium nitrite therapy for cyanide poisoning: concurrent use may reduce nitrite efficacy.
• High‑dose nitrous oxide exposure: can exacerbate functional B12 deficiency.

Moderate Interactions (Monitor Closely)

• Metformin: may lower serum B12; monitor levels during therapy.
• Proton pump inhibitors or H2‑blockers: may reduce oral B12 absorption, though less relevant for injectable form.
• Antacids containing aluminum or magnesium: monitor for potential additive effects on B12 status.
