Ibandronic Acid is indicated for bone‑resorptive disorders where excessive osteoclast activity leads to weakened skeletal structure.

• Osteoporosis: decreases the incidence of vertebral, hip and other non‑vertebral fractures by selectively inhibiting osteoclast‑mediated bone resorption.
• Paget's disease of bone: controls abnormal bone remodeling, reduces bone pain and lowers serum alkaline phosphatase levels.
• High‑risk patients with prior fracture: provides additional fracture risk reduction when combined with calcium and vitamin D supplementation.

• Ibandronic Acid binds to hydroxyapatite crystals on bone surfaces and is taken up by active osteoclasts. It inhibits the enzyme farnesyl pyrophosphate synthase, leading to osteoclast apoptosis and a marked reduction in bone resorption, which over time increases bone mineral density.

Ibandronic Acid may be associated with a range of adverse reactions, which are classified by frequency and severity.

Common Side Effects

• Gastrointestinal discomfort: nausea, abdominal pain or dyspepsia.
• Musculoskeletal pain: transient bone, joint or muscle aches.
• Flu‑like symptoms: fever, chills or fatigue after the first dose.

Serious or Rare Side Effects

• Osteonecrosis of the jaw: exposed bone in the oral cavity, often after dental procedures.
• Atypical femoral fractures: low‑energy fractures of the subtrochanteric or diaphyseal femur.
• Severe hypersensitivity: rash, angioedema or anaphylaxis.
• Renal impairment: acute kidney injury, especially with high oral doses.

• Renal function: assess creatinine clearance; avoid use in severe renal impairment.
• Calcium/vitamin D status: ensure adequate supplementation to reduce hypocalcaemia risk.
• Gastro‑intestinal disorders: use with caution in patients with esophageal strictures or active ulcer disease.
• Concomitant medications: review for potential interactions, especially with NSAIDs or nephrotoxic agents.
• Dental health: perform dental examination before treatment; postpone invasive procedures during therapy.
