Ibandronic Acid Teva (Ibandronic Acid)

Ibandronic acid is a bisphosphonate used to treat conditions involving increased bone resorption.

• Osteoporosis: reduces the risk of vertebral, hip and other fractures in postmenopausal women and men with osteoporosis.
• Osteoporosis in men: improves bone mineral density and lowers fracture incidence in male patients with primary osteoporosis.
• Paget's disease of bone: helps normalize abnormal bone remodeling in patients with Paget's disease.

• Ibandronic acid binds to hydroxyapatite crystals on bone surfaces, where it is taken up by osteoclasts during bone resorption. Inside osteoclasts it inhibits farnesyl pyrophosphate synthase in the mevalonate pathway, disrupting prenylation of small GTP‑binding proteins, leading to reduced osteoclast activity and decreased bone turnover.

Ibandronic acid may cause a range of adverse reactions, most of which are mild and reversible.

Common Side Effects

• Gastrointestinal irritation: dyspepsia, abdominal pain, nausea.
• Constipation or diarrhea.
• Musculoskeletal pain: bone, joint or muscle discomfort.
• Headache.
• Transient fever or flu‑like symptoms.

Serious or Rare Side Effects

• Osteonecrosis of the jaw (ONJ).

• Hypocalcemia.

• Calcium/vitamin D status: ensure adequate supplementation to reduce hypocalcemia risk.
• Concomitant NSAIDs or corticosteroids: monitor gastrointestinal tolerance.
• Dental health: advise dental examination before treatment and avoid invasive procedures during therapy.
• Administration technique: swallow tablet with a full glass of water, remain upright for at least 30 minutes.
• Store at room temperature: keep in the original packaging, protect from moisture and direct sunlight.

Major Interactions (Avoid)

• Calcium, magnesium, aluminum, or iron supplements taken within 2 hours of the dose (reduces absorption).
• Antacids containing aluminum or magnesium (impairs bioavailability).
• Severe renal impairment drugs (e.g., high‑dose aminoglycosides) that further compromise kidney function.
• Concurrent use of another bisphosphonate (duplicate therapy).
• Potent nephrotoxic agents (e.g., cisplatin) that may exacerbate renal risk.

Moderate Interactions (Monitor Closely)

• NSAIDs: increase risk of gastrointestinal irritation.
