Ibandronic Acid is a bisphosphonate that acts on bone‑resorbing cells to improve skeletal health.

• Osteoporosis in postmenopausal women: reduces the risk of vertebral and non‑vertebral fractures.
• Bone metastases (skeletal‑related events): lowers the incidence of pathological fractures, spinal cord compression and the need for radiation therapy.

• Ibandronic Acid binds selectively to hydroxyapatite crystals at sites of active bone remodeling. When osteoclasts attach to the drug‑coated bone surface, the compound is internalized, disrupting the mevalonate pathway and triggering apoptosis, thereby decreasing bone resorption and stabilising skeletal architecture.

• Acute phase reaction (fever, myalgia, arthralgia)
• Flu‑like symptoms
• Transient hypocalcaemia

• Renal function: assess baseline creatinine clearance and avoid use in severe renal impairment.
• Calcium status: correct hypocalcaemia and ensure adequate calcium/vitamin D intake.
• Dental health: obtain dental evaluation before treatment to reduce risk of osteonecrosis of the jaw.
• Concomitant medications: review other nephrotoxic or calcium‑affecting drugs.
• Pregnancy and lactation: avoid use unless clearly indicated by a physician.
• Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake.

Major Interactions (Avoid)

• Concurrent use of other bisphosphonates or potent nephrotoxic agents (e.g., aminoglycosides, NSAIDs) due to heightened renal risk.
• High‑dose calcium supplements administered within 6 hours of infusion, which can reduce drug absorption.

Moderate Interactions (Monitor Closely)

• Loop diuretics: may increase calcium loss, requiring electrolyte monitoring.
• Corticosteroids: can exacerbate bone loss, necessitating bone density assessment.
• Antacids containing aluminium or magnesium: may interfere with drug binding if given too close to infusion.
