Ibu-Ratiopharm (Ibuprofen)

Ibuprofen works by reducing inflammation and pain in several common conditions.

• Mild to moderate pain: provides analgesia for headaches, dental pain, musculoskeletal aches, and postoperative discomfort.
• Fever: lowers elevated body temperature associated with infections or inflammatory states.
• Inflammatory arthritis: helps control pain and swelling in osteoarthritis and rheumatoid arthritis.
• Dysmenorrhea: eases menstrual cramps and associated discomfort.
• Minor musculoskeletal injuries: reduces pain and swelling from sprains, strains and bruises.

• Ibuprofen inhibits cyclooxygenase‑1 (COX‑1) and cyclooxygenase‑2 (COX‑2) enzymes, decreasing the synthesis of prostaglandins that mediate inflammation, pain and fever. By limiting prostaglandin production at sites of tissue injury, it reduces vascular permeability, sensitization of nerve endings and the febrile response.

Adverse reactions may range from mild to severe, depending on individual susceptibility and duration of therapy.

Common Side Effects

• Gastrointestinal upset: nausea, dyspepsia or abdominal discomfort.
• Headache: transient mild to moderate headache.
• Dizziness: occasional light‑headedness or vertigo.
• Rash: mild skin irritation or pruritus.
• Elevated blood pressure: modest rise in systolic/diastolic values.

Serious or Rare Side Effects

• Gastrointestinal bleeding or ulceration.
• Severe allergic reactions: anaphylaxis, angioedema, or Stevens‑Johnson syndrome.
• Renal impairment: acute kidney injury or worsening chronic kidney disease.
• Hepatotoxicity: significant elevation of liver enzymes or hepatitis.
• Cardiovascular events: increased risk of myocardial infarction or stroke with long‑term high‑dose use.
• Hematologic abnormalities: thrombocytopenia or leukopenia.

Before initiating ibuprofen therapy, consider the following safety measures.

• Assess gastrointestinal risk: avoid in patients with active ulcers or a history of GI bleeding.
• Monitor renal function: use caution in those with impaired kidney function or dehydration.
• Use lowest effective dose: limit duration and dose to the minimum required for symptom control.
• Avoid in third trimester pregnancy: contraindicated due to risk of fetal ductus arteriosus closure.
• Caution with anticoagulants: increased bleeding risk when combined with warfarin or similar agents.
• Store at room temperature: keep in the original container, protect from moisture and direct sunlight.

Major Interactions (Avoid)

• Anticoagulants (e.g., warfarin): heightened risk of serious bleeding.
• Other NSAIDs or aspirin: additive gastrointestinal toxicity.
• Corticosteroids: increased chance of ulceration and GI bleeding.
• Lithium: ibuprofen may raise serum lithium levels, risking toxicity.
• Methotrexate: reduced clearance leading to toxicity.

Moderate Interactions (Monitor Closely)

• ACE inhibitors or ARBs: possible reduction in renal function.
• Diuretics: may exacerbate renal impairment.
• Selective serotonin reuptake inhibitors (SSRIs): modest increase in bleeding risk.
• Antihypertensives: potential attenuation of blood‑pressure‑lowering effect.
• Oral hypoglycemics: may affect glucose control in diabetic patients.
