Ifirmasta (Irbesartan)

Irbesartan is used to manage conditions related to the renin‑angiotensin system, particularly hypertension and diabetic kidney disease.

• Hypertension: lowers systolic and diastolic blood pressure, reducing cardiovascular risk.
• Diabetic nephropathy: slows progression of renal impairment in patients with type 2 diabetes.

• Irbesartan selectively blocks angiotensin II type 1 (AT1) receptors on vascular smooth muscle, adrenal gland and renal tubular cells. By preventing angiotensin II binding, it inhibits vasoconstriction, aldosterone release and sympathetic activation, leading to vasodilation, reduced sodium‑water retention and lower blood pressure, while also mitigating renal fibrosis in diabetic nephropathy. The blockade also improves endothelial function and decreases inflammatory mediators associated with hypertension.

• Hypotension: excessive drop in blood pressure leading to fainting.

Before initiating Irbesartan therapy, several precautions should be observed to ensure safety.

• Blood pressure monitoring: check baseline and periodic readings to avoid hypotension.
• Renal function assessment: evaluate serum creatinine and eGFR before and during treatment.
