Ilaxten (Bilastine)

Ilaxten, a second‑generation H1 antihistamine, is used to manage allergic conditions affecting the upper airway and skin.

• Allergic rhinitis: reduces nasal congestion, itching, sneezing and watery discharge.
• Chronic idiopathic urticaria: alleviates wheals and itching associated with hives.
• Seasonal allergic rhinitis: controls symptoms triggered by pollen or other seasonal allergens.

• Bilastine acts as a selective peripheral H1‑receptor antagonist. By binding to H1 receptors on mast cells and other target cells, it blocks the action of histamine released during allergic reactions, thereby preventing the cascade that leads to vasodilation, increased vascular permeability, and sensory nerve stimulation responsible for typical allergy symptoms.

Bilastine is generally well tolerated, but adverse reactions can occur.

Common Side Effects

• Headache: mild to moderate intensity.
• Drowsiness: occasional, may affect alertness.
• Dry mouth: transient sensation of oral dryness.
• Fatigue: feeling of reduced energy.
• Nausea: mild gastrointestinal upset.
• Dizziness: occasional light‑headed feeling.

Serious or Rare Side Effects

• Anaphylaxis: severe allergic reaction requiring immediate medical attention.
• Angioedema: swelling of face, lips, tongue or throat.
• Cardiac arrhythmia: irregular heart rhythm reported rarely.
• Severe skin reactions: such as Stevens‑Johnson syndrome.
• Hepatic injury: abnormal liver function tests.
• Blood dyscrasias: rare hematologic abnormalities.

Before using Ilaxten, consider the following safety measures.

• Renal impairment: dose adjustment may be required in patients with reduced kidney function.
• Hepatic impairment: use with caution in patients with liver disease.
• Driving and machinery: avoid operating vehicles if drowsiness occurs.
• Concomitant alcohol: limit intake as it may enhance sedation.
• Known hypersensitivity: do not use if allergic to bilastine or any excipients.
• Store at ambient temperature: keep below 25 °C, retain in original packaging, protect from moisture and direct sunlight.

Bilastine may interact with other medicines, affecting its efficacy or safety.

Major Interactions (Avoid)

• Strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin): may increase bilastine plasma levels.
• Other H1 antihistamines: concurrent use can enhance sedation and anticholinergic effects.
• Alcohol: combined use may intensify drowsiness.

Moderate Interactions (Monitor Closely)

• Fruit juices (apple, grapefruit): can reduce bilastine absorption, lowering effectiveness.
• Central nervous system depressants (e.g., benzodiazepines): may increase sedation.
• Anticholinergic agents: may potentiate dry mouth and constipation.
• Rifampicin: may decrease bilastine concentrations.
• Phenobarbital: may reduce therapeutic effect.
