Imakrebin (Imatinib)

Imakrebin (imatinib) targets malignant cells driven by specific tyrosine‑kinase pathways and is used in the following clinical settings:

• Chronic myeloid leukemia (CML): provides disease control by inhibiting BCR‑ABL‑positive leukemic cells.
• Gastrointestinal stromal tumor (GIST): induces tumor shrinkage through c‑KIT inhibition.
• Philadelphia chromosome‑positive acute lymphoblastic leukemia (Ph+ ALL): offers targeted suppression of BCR‑ABL activity in lymphoid blasts.

• Imatinib binds competitively to the ATP‑binding site of BCR‑ABL, c‑KIT, and PDGFR tyrosine kinases, stabilising them in an inactive conformation. This blocks downstream signaling cascades that drive cell proliferation, survival, and angiogenesis, leading to apoptosis of malignant cells that depend on these kinases for growth.

Imakrebin may cause a range of adverse reactions that vary in frequency and severity.

Common Side Effects

• Nausea: mild to moderate upset stomach.
• Edema: swelling of extremities, especially ankles.
• Fatigue: persistent tiredness.
• Diarrhoea: loose stools.

Serious or Rare Side Effects

• Neutropenia: marked reduction in white blood cells.
• Hepatotoxicity: elevated liver enzymes or liver injury.
• Severe skin reactions: Stevens‑Johnson syndrome or toxic epidermal necrolysis.

When prescribing Imakrebin, clinicians should observe several safety measures.

• Blood Count Monitoring: perform regular CBC to detect neutropenia.
• Liver Function Assessment: check hepatic enzymes before and during therapy.
• Cardiac Evaluation: assess baseline cardiac status in patients with pre‑existing heart disease.
• Pregnancy Consideration: avoid use unless benefits outweigh potential fetal risks.
• Hypersensitivity Warning: discontinue if severe allergic reaction occurs.
• Store at below 25°C: keep in the original packaging, protect from moisture and direct sunlight.

Imakrebin interacts with several medicines that can alter its efficacy or safety.

Major Interactions (Avoid)

• Strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole): may increase imatinib plasma levels.
• Strong CYP3A4 inducers (e.g., rifampicin, carbamazepine): may reduce therapeutic exposure.
• Antacids containing aluminium or magnesium: can impair absorption if taken simultaneously.
