Imatek (Imatinib)

Imatek (imatinib) is indicated for malignancies driven by BCR‑ABL or KIT/PDGFRA tyrosine kinase activity.

• Chronic myeloid leukemia (CML): reduces leukemic cell proliferation and improves long‑term survival.
• Philadelphia chromosome‑positive acute lymphoblastic leukemia (Ph+ ALL): helps achieve remission and extends disease‑free intervals.
• Gastrointestinal stromal tumor (GIST): inhibits tumor growth and delays disease progression.

• Imatinib selectively binds to the ATP‑binding pocket of BCR‑ABL, KIT and PDGFRA tyrosine kinases, stabilizing the inactive conformation of these enzymes. By preventing phosphorylation of downstream substrates, the drug blocks signaling pathways that drive uncontrolled cell division and survival, thereby suppressing the proliferation of malignant clones in affected tissues. The inhibition is dose‑dependent and results in apoptosis of the transformed cells while sparing most normal hematopoietic cells.

Imatinib may cause a range of adverse reactions, varying in frequency and severity.

Common Side Effects

• Nausea: mild to moderate upset stomach.
• Diarrhea: loose stools, often transient.
• Edema: swelling of the ankles or peripheral tissues.
• Fatigue: generalized tiredness or weakness.

Serious or Rare Side Effects

• Hepatotoxicity: elevated liver enzymes or hepatitis.

• Severe neutropenia: marked decrease in white blood cells increasing infection risk.
• Pulmonary hypertension: increased pressure in lung arteries that may cause shortness of breath.

• Baseline assessment: obtain complete blood count, liver function tests and cardiac evaluation before starting therapy.
• Monitoring: perform regular CBC, hepatic panel and ECG during treatment to detect toxicity early.
• Drug interactions: avoid concomitant use with strong CYP3A4 inhibitors that may increase plasma levels.
• Pregnancy and lactation: assess risk‑benefit; use only if potential benefit justifies potential fetal or neonatal exposure.
• Patient counseling: advise patients to report persistent edema, unexplained bruising, or signs of infection promptly.
• Store at ambient temperature: keep tablets in original container, protect from moisture and light, and store below 25 °C.
