Imaxi (Amoxicillin)

Amoxicillin is used to treat bacterial infections affecting several organ systems.

• Respiratory tract infections: helps eradicate the bacterial pathogens causing acute bronchitis, pneumonia and related conditions.
• Otitis media: clears middle‑ear infections by eliminating susceptible streptococci and Haemophilus species.
• Sinusitis: reduces sinus inflammation by targeting the causative bacteria.
• Urinary tract infections: treats uncomplicated UTIs by inhibiting growth of common uropathogens.
• Skin and soft tissue infections: resolves cellulitis and wound infections caused by susceptible Gram‑positive organisms.

• Amoxicillin exerts its antibacterial effect by binding to penicillin‑binding proteins located on the bacterial cell wall. This binding interferes with the final stages of peptidoglycan cross‑linking, weakening the cell wall and leading to osmotic lysis of susceptible bacteria. The drug is stable against many β‑lactamases, allowing activity against a broad spectrum of Gram‑positive and some Gram‑negative organisms. By inhibiting cell wall synthesis, it is bactericidal against actively dividing organisms.

Amoxicillin may cause a range of adverse reactions, from mild to severe.

Common Side Effects

• Nausea: mild stomach upset that often resolves without treatment.
• Diarrhea: loose stools that may be self‑limiting.
• Rash: skin redness or itching without systemic involvement.
• Vomiting: occasional emesis, usually transient.
• Taste disturbance: altered taste perception during therapy.

Serious or Rare Side Effects

• Anaphylaxis: severe allergic reaction with airway swelling, requiring immediate medical attention.
• Clostridioides difficile‑associated diarrhea: severe colitis that may occur during or after therapy.
• Stevens‑Johnson syndrome: rare skin blistering and mucosal involvement.
• Serum sickness–like reaction: fever, joint pain and rash after exposure.
• Hepatic injury: elevated liver enzymes or jaundice.
• Blood dyscrasias: neutropenia, thrombocytopenia or hemolytic anemia.

Before prescribing amoxicillin, clinicians should consider several safety precautions.

• Allergy assessment: confirm no known hypersensitivity to penicillins or β‑lactam antibiotics.
• Renal function: adjust dose in patients with impaired kidney function to avoid accumulation.
• Hepatic impairment: monitor liver function tests in patients with existing liver disease.
• Concomitant antibiotics: avoid use with other β‑lactams that may increase risk of toxicity.
• Pregnancy and lactation: weigh potential benefits against unknown risks, as safety data are limited.
• Store at ambient temperature: keep below 25 °C in the original packaging, protected from moisture and light.

Major Interactions (Avoid)

• Probenecid: reduces renal excretion of amoxicillin, markedly increasing plasma concentrations.
• Methotrexate: may enhance methotrexate toxicity by decreasing its clearance.
• Oral contraceptives: may reduce hormonal efficacy, increasing risk of unintended pregnancy.

Moderate Interactions (Monitor Closely)

• Warfarin: amoxicillin can potentiate anticoagulant effect; monitor INR regularly.
• Allopurinol: combined use may increase risk of skin rash; observe for dermatologic reactions.
