Imigran (Sumatriptan)

Imigran (sumatriptan) is indicated for the acute treatment of migraine, a neurovascular headache disorder.

• Migraine with aura: provides rapid relief of headache and associated neurological symptoms.
• Migraine without aura: reduces headache intensity and duration when taken early in an attack.
• Migraine when oral administration is not feasible: nasal spray offers an alternative route for quick absorption and symptom control.

• Sumatriptan acts as a selective agonist at serotonin 5‑HT1B and 5‑HT1D receptors. Activation of these receptors produces cranial vasoconstriction and inhibits the release of pro‑inflammatory neuropeptides from trigeminal nerve endings, thereby interrupting the pain pathway responsible for migraine attacks.

Common Side Effects

• Nasal irritation or burning sensation.
• Unpleasant taste or metallic taste.
• Dizziness or light‑headedness.
• Somnolence or mild fatigue.
• Nausea or mild stomach upset.
• Throat irritation.

Serious or Rare Side Effects

• Chest pain or tightness suggesting coronary vasospasm.
• Severe hypertension or hypertensive crisis.
• Serotonin syndrome, especially with other serotonergic agents.
• Allergic reactions including rash, angioedema, or anaphylaxis.
• Ischemic events such as myocardial infarction or stroke.
• Severe arrhythmias or cardiac rhythm disturbances.

Imigran should be used with caution in patients with certain medical conditions and in specific situations.

• Cardiovascular disease: avoid use in patients with known coronary artery disease, uncontrolled hypertension, or history of myocardial infarction.
• Serotonergic agents: assess risk of serotonin syndrome when combined with SSRIs, SNRIs, MAO inhibitors, or other triptans.
• Pregnancy: safety not established; use only if potential benefit justifies potential risk to the fetus.
• Elderly patients: start with the lowest effective dose and monitor for adverse reactions.
• Drug interactions: review all concomitant medications for possible interactions before prescribing.
• Store at room temperature: keep in the original container, protect from moisture and direct sunlight.

Major Interactions (Avoid)

• MAO inhibitors (e.g., phenelzine, tranylcypromine): risk of severe hypertension.
