Imunocare (Imiquimod)

Imiquimod acts on skin lesions by activating innate immune pathways, making it useful for several dermatologic conditions.

• Actinic keratosis: reduces and eliminates precancerous lesions on sun‑exposed skin.
• Superficial basal cell carcinoma: promotes clearance of early‑stage basal cell tumors.
• External genital warts: induces regression of HPV‑related warty growths.

• Imiquimod is a Toll‑like receptor 7 (TLR7) agonist that activates dendritic cells and macrophages, leading to the production of interferon‑α, tumor necrosis factor‑α, interleukins and other cytokines. This cytokine cascade enhances both innate and adaptive immune responses, resulting in antiviral activity and the destruction of dysplastic or neoplastic skin cells. The localized immune activation also promotes apoptosis of abnormal keratinocytes and facilitates lesion resolution.

Topical imiquimod may cause skin reactions at the application site and, rarely, systemic effects.

Common Side Effects

• Redness: localized erythema at the treated area.
• Itching: mild to moderate pruritus.
• Burning sensation: transient heat or stinging.
• Pain: discomfort during treatment.
• Swelling: mild edema around the lesion.
• Crusting or scaling: formation of dry scabs.

Serious or Rare Side Effects

• Severe allergic reaction: angioedema or anaphylaxis.
• Extensive skin ulceration: deep tissue breakdown.
• Systemic flu‑like symptoms: fever, chills, fatigue.
• Secondary infection: bacterial overgrowth of the lesion.
• Persistent hyperpigmentation: lasting darkening of skin.

Before using Imunocare, consider the following precautions to minimize risks and ensure safe application.

• Application area: apply only to intact skin lesions as directed by a healthcare professional.
• Occlusion: do not cover the treated area with dressings unless specifically instructed.
• Concomitant skin products: avoid using other topical medications on the same site.
• Systemic symptoms: discontinue use and seek medical advice if fever, malaise, or lymphadenopathy develop.
• Pregnancy and lactation: safety not established; avoid use unless the potential benefit justifies the risk.
• Store at room temperature: keep the tube in the original packaging, protect from moisture and light.

Imiquimod has limited systemic absorption, but certain co‑administered agents may affect its safety or efficacy.

Major Interactions (Avoid)

• Other topical immunomodulators: concurrent use may increase local irritation.
• Systemic immunosuppressants: may reduce the therapeutic response of imiquimod.

Moderate Interactions (Monitor Closely)

• Antiviral agents (e.g., acyclovir): monitor for overlapping skin reactions.
• Phototherapy: increased risk of skin irritation when combined.
• Topical corticosteroids: may diminish imiquimod’s efficacy if used simultaneously.
