Indapen (Indapamide)

Indapamide is a thiazide‑like diuretic that acts on the renal distal convoluted tubule to manage fluid‑related conditions.

• Hypertension: lowers systemic blood pressure by reducing extracellular fluid volume and peripheral vascular resistance.
• Edema associated with heart failure: decreases fluid overload, improving symptoms and exercise tolerance.
• Edema due to renal or hepatic disease: facilitates removal of excess interstitial fluid, helping to control swelling.

• Indapamide inhibits the Na⁺/Cl⁻ cotransporter in the distal convoluted tubule, reducing sodium and chloride reabsorption. This leads to increased urinary excretion of water and electrolytes, decreasing plasma volume. The resultant reduction in intravascular volume, together with a direct vasodilatory effect on peripheral arterioles, lowers peripheral vascular resistance and contributes to blood pressure reduction.

Indapamide may cause a range of adverse reactions, from mild to severe.

Common Side Effects

• Dizziness: sensation of light‑headedness, especially after standing.
• Headache: mild to moderate pain in the head region.
• Increased urination: more frequent need to void.
• Electrolyte imbalance (e.g., low potassium): may cause muscle weakness or cramps.

Serious or Rare Side Effects

• Severe dehydration: excessive fluid loss leading to hypotension.
• Significant electrolyte disturbances (e.g., hyponatremia, hyperuricemia): can precipitate cardiac or neurological issues.
• Allergic reactions: rash, itching, or swelling of the face and throat.
• Pancreatitis: inflammation of the pancreas presenting with abdominal pain.
• Blood dyscrasias: rare blood cell abnormalities such as agranulocytosis.

• Monitor electrolytes: regularly check serum potassium, sodium, and uric acid levels.
• Assess renal function: evaluate creatinine clearance, especially in patients with existing kidney disease.
• Use caution in gout: thiazide‑like diuretics can raise uric acid concentrations.
