Indom (Indometacin)

Indometacin ophthalmic is used to manage inflammatory conditions of the eye by inhibiting prostaglandin synthesis.

• Post‑operative ocular inflammation: diminishes swelling and discomfort after eye surgery.
• Allergic conjunctivitis: relieves redness, itching and swelling caused by allergic reactions.
• Anterior uveitis: helps control inflammation of the front part of the eye.

• Indometacin inhibits cyclooxygenase (COX‑1 and COX‑2) enzymes in ocular tissues, leading to decreased conversion of arachidonic acid to prostaglandins. The reduction in prostaglandin levels limits inflammatory mediators, thereby lowering pain, edema, and cellular infiltration in the eye.

Indometacin ophthalmic may cause local and systemic adverse reactions.

Common Side Effects

• Burning or stinging sensation: transient discomfort upon instillation.
• Blurred vision: temporary visual disturbance that resolves.
• Redness of the conjunctiva: mild irritation of the eye surface.
• Tearing: increased lacrimation after dosing.

Serious or Rare Side Effects

• Corneal ulceration: rare but severe damage to the corneal surface.
• Anterior uveitis exacerbation: worsening of intra‑ocular inflammation.
• Systemic gastrointestinal ulceration: possible if significant systemic absorption occurs.
• Renal impairment: rare renal function changes in susceptible patients.

Indometacin eye drops require careful use to minimise risk.

• Hypersensitivity: avoid if patient has known allergy to indometacin or other NSAIDs.
• Corneal disease: use with caution in patients with epithelial defects or keratitis.
• Pregnancy and lactation: safety not established; consider alternative therapy.
• Systemic NSAID use: monitor for additive gastrointestinal or renal effects.
• Renal insufficiency: assess renal function before initiating therapy.
• Store at room temperature: keep in original container, protect from light and moisture.

Indometacin ophthalmic can interact with systemic and ocular agents.

Major Interactions (Avoid)

• Systemic anticoagulants (e.g., warfarin): increased bleeding risk.
• Systemic corticosteroids: heightened risk of gastrointestinal ulceration.

Moderate Interactions (Monitor Closely)

• Other topical NSAIDs: additive ocular irritation.
• Beta‑blocker eye drops: possible reduction in intra‑ocular pressure control.
• Diuretics: monitor renal function if systemic absorption is significant.
